When compared to https://www.wowhead.com/mop-classic/item=49975 you swap the 60 spirit for 30 mp5 but give up the socket in exchange for more crit and spellpower. Interestingly enough this is the only known neck piece from ICC-25 that does not have a socket. Unless you are a haste-capped healer you should be looking into something different, for instance https://www.wowhead.com/mop-classic/item=50809 is better than this piece (if you aren't in desperate need of the mp5) even as an ilvl 251.

As a side note, I wonder since this is a PTR drop if it's been fixed to add a socket on live? If so the worth should increase.

this peice of loot drops off Festergut but also has a yellow gem slot.

The name is most likely a reference to the extended time it took to open The Plagueworks when compared to the other wings of the instance.
