WARNING!
Do not transmog this weapon. Right now, you could loot this hammer and have this item nerfed to ilvl 125 for no reason. Both our raiders got their hammer nerfed. Actually, there seem to be an issue where the item dropped is Khor, Hammer of the Guardian. That item was 355 last night and got put to 125. They both transmogged their weapon, so maybe it's that. Either way, we sent a ticket to report the bug. Basically the weapon is useless until Blizz revert that weird bug!

NOTE: There are 2 similar hammers, Khor, Hammer of the Guardian & Khor, Hammer of the Corrupted.

It appears (now confirmed) that 1 is a transmog only item "Khor, Hammer of the Guardian", the other is an actual weapon "Khor, Hammer of the Corrupted". The problem is/was, the transmog only item showed up as the wrong item level and once you transmogged it, enchanted it, or relogged (I believe), the item level then showed correctly so you thought it lost levels, (although this has now been hot fixed to show the correct item level when it drops).

UPDATE: For those who got the the other hammer before the hotfix (that corrected the transmog item showing the wrong item level), and thus making some think they had an actual weapon, there is a fix just for you all! https://us.battle.net/forums/en/wow/topic/20768687371?page=11#post-215

Transmog version: Khor, Hammer of the Guardian (not listed in the loot table)
Both can be dropped by Taloc.
