Tier 16 is a 5-piece set obtained with tokens from Assedio di Orgrimmar.
There are three different colors: 528 gear from LFR, 540 from Flex shares colors with 553 from Normal, and 566 from Heroics. This is the heroic colored set.
These tokens are turned into tier from vendors at your faction's shrine:
Alliance: Lorry Cuorfocoso (Heroic)
Horde: Tu'aho Tagliavie (Heroic)

Tier drops are as follows:

Helm: Heroic tokens from Thok il Sanguinario (Wing 3) in Assedio di Orgrimmar.
Shoulders: Heroic tokens from Ingegnere d'Assedio Miccianera (Wing 4) in Assedio di Orgrimmar.
Chest: Heroic tokens from Sha dell'Orgoglio (Wing 1) in Assedio di Orgrimmar.
Gloves: Heroic tokens from Generale Nazgrim (Wing 2) in Assedio di Orgrimmar.
Legs: Heroic tokens from Klaxxi Paragons (Wing 4) in Assedio di Orgrimmar.

In addition, Garrosh Malogrido drops a special tier token that can be redeemed for any slot, like Essenza del Conquistatore Maledetto.
