Before patch 2.0, there was 14 PvP titles you could advance through. Commander was the 11th, for alliance players. The horde version of this title was Lieutenant General. Getting this title also rewarded an epic mount.

After 2.0, these titles were removed, and could no longer be obtained. Those of you who are using the wowhead launcher, will be able to see all the 28 titles (yes, horde AND alliance) as "incomplete" titles in the profiler.

I got this one on my hunter toon. And Netspook is correct, this is nr. 11 of 14. The only sad thing is, that after the mounts became consumable, you cannot see the difference between the original Commander's mount and the "fake copies" everyone else runs around with. the original said: "requires level 55", while all these other copies were "requires level 60"

This is the hottest title in wod, as all the npcs especially in your garrison keeps referring to you as commander.
