You can get the sample from the Irradiated Slime in the dungeon... You know, the big green blobs.

After you enter the instance, follow the hallway straight down until you have to go left or right. Take a left. Pay attention... Follow that hallway all the way down and take your second left. That last left will take you into the "Dormitory." After you've followed the Dormitory hallway all the way around and into a larger room, behind the wall directly across from where you just came out is "The Hall of Gears." In that room are the Irradiated Slime. Don't attack them, just get close and right click the vial. As the quest says, it may take a few tries!
