The flavor text is clearly a reference to Super Mario (the classic games). When you completed a castle back then, some toad would say (I may be misquoting here, but cut me some slack):

Thanks for saving me/us, Mario! Unfortunately, the princess is in another castle!

As far as I know, the code you get will grant you a pet on Live realms.

Appear to drop off of bosses in Ulduar on the PTR, though they may have been removed off loot tables for bosses that have had their loot added in.

Random fact: as of August 20th, 2009, this is the only item above blue quality that has an item level between 164 (Kil'jaeden drops) and 200 (Entry-level WotLK epics from reputation, heroics, Naxx 10-man, and Obsidian Sanctum 10-man)
