You recieve this by walking through a sphere while phase diving. These spheres have some spawn locations and can be spawned when killing mobs. Collecting all phase-lost transmogs will reward you with an achievement Oggetti di fase smarriti (<- here you find screenshots of the orbs and some spawn locations in the comments) from which you recieve a mount Scistodorso Fuori Fase.

To any warlocks wondering if you can mail the 2h weapons to an alt and then continue collecting orbs to get a second copy, the answer is yes.
Just make sure not to "learn" any 2h you want to use with Glyph of Felguard. Mail them immediately to an alt and continue collecting orbs and eventually you'll get them again from the orbs.

Not sure but there's a good chance that once learned you can never get another copy of these weapons.
