So coords being posted aren't working correctly. Go to Scarlet Monastery (outdoors), walk up the stairs into The Grand Vestibule, turn left, and left, into a cloister with 2 Scarlet mobs, and 3 candelabras. It's on the wall, behind the middle candelabra with the Scarlet Crusade banner on the wall (you should be immediately facing it as you enter the small side room).

Not sure if this has anything to do with it but thought I should mention it here:
Had trouble finding the rune at the correct location, nothing was there. Tried switching to Pre-BFA Tirisfal and went to the same room, found it there this time.

It's inside the Scarlet Monastery building in Tirisfal Glades (not inside the instance).

As of writing this, the coordinates in the Wowhead guide had the wrong zone ID (18 instead of 19). The correct coordinates are:
/way #19 40.62 52.52

It's behind the middle candle in a little alcove in the middle of the banner and is very hard to see.

Go up the steps to the right, once at the top go left, and left into the first room you see.

Back of the room, behind the candle stick on the tapestry you will find "Carved Eye" Location 40, 52 INSIDE the Scarlet Monastery
