This item is the final reward of a little secret in the dungeon journal. The entire sequence takes five weeks to complete.

Each week (after reset), open your adventure journal (shift+j) and scroll all the way to the right. You'll get one of these quests, in order:

Week 1 - Una moneta di rame - Rewards 1c and the achievement Monetina fortunata.
Week 2 - Una moneta d'argento - Rewards 1s
Week 3 - Una moneta d'oro - Rewards 1g
Week 4 - Una borsa d'oro - Rewards 10g
Week 5 - Una strana moneta - Rewards Moneta Scarabocchiata d'Ombra, the achievement Strana moneta, and a message from our mysterious benefactor.

If you'd like to know what the final message says, I've posted it on Twitter since Wowhead doesn't have it in the database yet.
