Soffio della Serpe Immortale, which unlocks the Breath of the Undying Serpent hidden appearance for the Mistweaver Artifact weapon (Sheilun, Bastone delle Nebbie), is dropped from the Dragons of Nightmare encounter in Incubo di Smeraldo Legion raid instance.
This is an encounter where you face off against Ysondre and the nightmares of her former cohorts, Emeriss, Lethon and Taerar.

Some notes on this item:

This item is not a guaranteed drop.
The item has a chance to drop if there are ANY Monacos in the raid, regardless of their spec or loot spec.
It drops on LFR, Normal, Heroic or Mythic difficulties.
If the loot setting is Master Loot, it is like any other loot drops – the Master Looter determines who to assign the item to.
If the loot setting is Personal Loot, it has a chance to be looted from the boss like any other gear and can drop for ANY spec of Monaco.
Regardless of loot settings, (Master Loot or Personal Loot) this item can be traded to other members of the raid for up to 2 hours after it is obtained.

Can say that this ISN'T a sure drop

I received this on my first Normal mode kill as Windwalker with Windwalker also as my loot spec.

For those that already have the hidden appearance, Here's how to track your hidden artifact appearance color variants through achievement progress:

/run AddTrackedAchievement(11152) - Complete 30 dungeons using a hidden artifact appearance. (This achievement does not display correctly)
/run AddTrackedAchievement(11153) - Complete 200 world quests using a hidden artifact appearance.
/run AddTrackedAchievement(11154) - Kill 1,000 players using a hidden artifact appearance.

Check all your hidden artifact appearance progress using this script
/run local q,x,_,a,b = GetAchievementCriteriaInfo,0 for i=1,11 do _,_,_,a,b = q(11152,i) x=a end local _,_,_,c,d = q(11153,1) local _,_,_,e,f = q(11154,1) print("Dungeons: "..x.."/"..b) print("WQs: "..c.."/"..d) print("Kills: "..e.."/"..f)

Note: Completing any of these achievements will complete them for all your other specializations on that character, regardless of having unlocked the consumable items for your other specs.

Can tell you, you dont need to have right Spec speciala, i did playing MW on normal dif, but WW speciala.

First time i killed him in raid finder it dropped. I was specced as a mistweaver

I can confirm this is a very high droprate, though not 100% it can drop on ANY Difficulty, LFR, Normal, Heroic or Mythic, regardless of your spec/Loot pref.

Just got mine to drop out of LFR today, so I can confirm that it does drop out of that difficulty!

Just recieved this from my first mythic kill. We were using Master Looter and it was given out to me.

This appearance may also be traded to other players (in the same raid group, of course). It dropped to a lovely player playing WW, and s/he gave it to me.
Keep an eye out for other Monks' loot after the encounter; goodluck! :)

And as a slight update.... between 2 monks I've killed this boss 33 times on 3 difficulties. NOT a high drop rate unfortunately.

10/10 my favorite artifact skin for Mistweavers! I love the fact that it has that eastern look to it, but the dragon featured on the staff is not one of the cloud serpents. Instead it's an emerald dragon, which does make sense since they're known for their ability to heal. The overall design is just beautiful - and I am afraid that it will take me a while to get it.. but most likely #worth it.

I've killed this on lfr, normal, heroic, and mythic every single week. no drop anywhere. Random Mw killin it their first time gets it... whats wrong with me man

Got this today on my second run. I did LFR once and then normal. It dropped on normal.

I was in an LFR group with 3 monks (one of them was me), and 2 out of 3 of the monks got the appearance. Want to guess which Monk I was? I did a Normal mode right afterwards and still nothing. Let's just say that I'm not very lucky apparently.

Something that hasn't been explicitly mentioned here: it has a chance to drop on each difficulty, even if you have already done the Dragons on another difficulty this week - I did the raid, in order, on LFR, Normal, Heroic, and Mythic, and got this item on the Mythic run.
