I can't give an exact incident (anymore, been too many) but going in a PuG with a less-than competent tank who can't hold aggro (I don't have silent resolve) .. if there are multiple adds rofl, and I see the tank has 100 rage and I get aggro, I know it's going to be a looooooong instance. An example... UP gauntlet. x.o
Hmm, I could give a name of a tank I used to group with all the time in TBC that did exactly that...Took me a while of constantly forgetting his name to put him up on my friends list with a comment something like "fail tank."

I seem to run into them problems a lot, with pug tanks. They pull, I'm at 20% mana, I clearly stated "wait for mana", so I sit back watch them all die. They usually start complaining that it is all my fault, in reply "If you don't wait for mana, you don't get heals."
Back in TBC I had one tank that pulled 5 or 6 times when I no next to no mana, after the second time all the dps in the group just sat by me and watched him die, Was quite an interesting heroic Arc run.

Besides tanks and dps may have a friendslist too where you would end up on as "fail healer" ;)

Doing a Nexus PuG. Pally tank, Rogue, Hunter, Mage, and Me (Holy).
The pally was wearing outland quest greens and a few blues. I decided, eh, whatever.
So we get started. The pally eats a bit more damage than I'm used to (naturally), but that's not really a problem. The problem is that he can't hold aggro. At all. Everyone's got agg, every pull. So we wiped disgustingly early in the dungeon.
We revive and continue. A bit further in Hunter gets frustrated. He's resorted to
autoshooting alone
to try to prevent pulling agg, and his agg management abilities are all well and good but aren't meant to be used every moment in a dungeon. Even autoshooting, he's pulling agg. So he calls Pally out, though refrains from using any inciting language. Everyone stays calm and we continue.
We get to Grand Magus Telestra. We don't wipe, but I die keeping everyone else up and, tbh, not paying enough attention to my own bar...oops. Pally is quick to rez me and pop his buff back on me. I set myself up to regain mana and rebuff myself. The pally immediately runs out and around the corner to the corridor across from Telestra's room.
My guild runs to that spot to prepare for the next pull. They stand against the wall over there and don't do anything stupid. So I thought that's what they were doing. Well, it's what the DPSers were doing. And besides, Pally
just rezzed the healer
so obviously he's not going to---he pulled. I was...sitting in the middle of Telestra's room at half mana, without buffs. I know, I should have stayed put. Anyway, ran too close to the mobs they'd skipped (the ones right outside the entrance to her room) in my attempt to get to them.
So we wiped, and I helped =(
Then Rogue realizes something.
Rogue - "Hey, why are you using Seal of Justice? Please switch to Seal of Righteousness."
Pally - "I shouldn't use Righteousness.
The stuns do more threat than holy damage does
." <-- Things you never want to hear from your tank...*facepalm*
Rogue - "Please try it."
Rogue then starts PMing me about the Pally, LoL.
Thankfully, Pally isn't too stubborn to switch. So now he's holding threat...well, better. It's still awful, but now it's 3 people having agg all the time instead of 5. Rogue asks how Pally's words taste and Pally thankfully has the good grace to reply "Tasty!" instead of getting into something. We wipe less and eventually get to Keristrasza.
I pride myself on not losing people on Keristrasza...but that just wasn't happening on this run.
At the end it was me and the Rogue. But everyone started celebrating because we actually managed to clear the damned thing with everyone having agg on every pull, LoL.
But I will never forget hearing "The stuns do more threat."

It's Pally Tanks who have been the most trouble for me. Just once I want to play with a Pally tank who knows how to hold Aggro. I smile when I get a Warrior tank, and a DK can go either way. But Pallies make me sad.
That pally, though he was a horrible, horrible tank...seemed to be open-minded enough to be willing to learn.

If the tank is THAT bad, I wouldn't doubt that he ebay'd his account. No doubt at all.
And yeah, I can agree that it's really frustrating when the tank dies due to Mortal Strike, and the healers get bashed for not healing well enough. Even more upsetting is in Spider Wing in Naxx, when you're a healer that can't remove Curses, and the tank(s) get the -75% healing curse on trash, and die because nobody removed it. Same thing on Maexxna, when the druid/paladin/shaman doesn't cure the poison fast enough and the healers get blamed for it.

I have the opposite problem in Heroics. I clearly state that I will always have mana, no matter what you see, I will heal you. Tanks don't seem to understand that even at <1000 mana I have a Shadowfiend and Hymn of Hope in wait to keep me topped off...and I only have that little mana on groups that should be very grateful for having an OP healer.
I get things like:
DPS: were rdy, y u not pulln?
Tank: waitin on heals mana
Me: I'm fine, start pulling.
Tank: u got no mana
Me: You'll get healed; don't worry. I have enough mana.
*Finish the trash pull...I'm sitting at 100% mana. (I only wish I could see the look on the tank's face atm)
Yes, it's nice to be OP and be able to heal those heroics where 5 people pull aggro consistently. However, even in a 10-man, I expect more from my tanks. However...that wasn't always the case obviously. I shudder to remember the days where I needed a proper tank...and when I didn't get what I needed.
