I believe current gems and items will remain unchanged. Then the expansion will introduce the new itemization - items will start having considerably higher primary stats (Strength, Agility, Intellect, Spirit), and no secondary stats (AP, SP, ArP, MP5, etc). The higher values on primary stats will give you roughly the same benefit as lower value on primary, but some on secondary as well.
Of course, future AP, SP, ArP, and MP5 gems won't be needed, as they don't fit the itemization.

Anyways, on topic now, I think they'll just convert all gear/gems with Spellpower or any other stats being removed to have the new stats, like say, a Hat with Mp5 and Spellpower, but no other stats, will simply be changed to have just Intellect and Spirit.

I think nothing like that will happen. If they convert an SP hat into an Intellect one, they will be giving not only SP, but Mana and Crit as well to it. I think current items will stay the same - after all

What about the gems that changed (ie: Teardrop) which gave +Healing. When the patch that came out that changed it all to +Spell power that gem changed also. Will the demise of AP/Spell Power/MP5 also change the current gems to new(relevant) stats?

Useless and terrible, yes, obsolete? No. What I mean by that, is that while that piece of gear is horrendous for Paladins to wear, but it still fell in the rules of itemization. I have a feeling that it's not going to stay that way when 4.0 arrives. I think the spell power on it will change to intellect, thereby fitting it to the new itemization standard. I also believe that's what's going to happen to gems, which, as Boomrocket pointed out, happened with +healing and +spell damage gems in 3.0.
These things remains to be seen though, should be exciting when it happens :)

I'm sure they've thought of it. And remember, they have over a year to work out the bugs. My guess is we'll have something like +16 int red gems and yellow gems for northrend (all sp gems changing to int) and the removal of the +16 int red gems in Cat gemming. It's sort of like how occasionally outlands quest reward choices will feature two identical pieces of spell caster gear. That was from back when +healing and +damage were different stats. Once they combined them, some of those rewards were identical, and they left it like that, but subsequent gear took these differences into account, so you never see that thing happening in northrend quest rewards.
