To craft these, the Altar of Decay is required. The Altar of Decay is located in the dungeon Brackenhide hollow. The altar is surrounded by a poison that does damage and silences you. You will have to kill the last boss in order to clear this poison and access the altar. The altar is above the last boss (go up on the right side).

Can the recipe drop from the area outside of the dungeon marked Brackenhide Hollow? Tooltip is saying "surroundings" so looking to confirm.

Horrible. You have to fight through the whole dungeon just to complete a crafting order for this item. Blizzard needs to relocate the Altar or put another one available in open world.
