Anduin wasn't named king for the same reason Varian wasn't actually named king till he was 18, it's an age thing. That's why Bolvar/Prestor were the Reagant lords till Anduin came of Age.
As for Kael, it's possible he didn't want to be King, or he was in mourning for his father. We don't know much about the rules the Blood Elves use after all, it could be, if the King dies in battle, there is a year or more of Mourning before the Heir takes the throne.
