Trouble Tanking with Multiple Targets

For all you expert tanks out there, i need some of your guidance on this one.
I've been trying to tank Heroic instances with some success. However, my only problem is tanking multiple targets, ie: 3+ targets. I can't seem to have the mob maintain aggro on me. I can maintain 2, but sometimes the 3rd or even the 4th target(s) seem to escape me and either head for the healer or another member in the party.
This is what i currently do.
- I charge into the mob, Thunder clap, Demo Shout the mob. Devastate the first target to 4 (2 clicks - Glpyhed up), tab to next target, devastate to 4, and repeat with other targets.
- I'll go back to my first target, devastate, and throw in a shield slam (after shield block, if up) and move to the next target, then i'll use revenge (if up) or heroic strike, or whatever is up to generate threat. I'll keep repeating.
- I'll throw in concussion blow, shockwave, mocking blow to some targets, but if they're on cooldown before the next mob fight, I won't be able to use them until mid-fight
- I'll renew demo shout and thunder clap up if they're about to expire.
- I'll use taunt for that 1 target that escapes, but i have to run to them if they're too far.
- If two or more targets escape, i'll use challenging shout.
so what am i doing wrong? is it the dpsers and healer generating more threat than me?
should the party ALWAYS take down 1 target at a time?

I didn't see shockwave in any of your rotations...
Might help.
Also, if need be use cc again! God knows its been kicked down to nothing, but people still do it.

As a long time tank..let me give you some decent advise. Dual spec tanking. One for trash mob and one for Bosses. Speccing impale/deep wound is not necessary during trash mob fights. Your job is to just hold the mob together. Get the glyph of cleave and shockwave(reduces cooldown) and Resonating Power(reduces thunderclap rage by 5 points+plus 3 point in improve shockwave and focus rage means your thunder clap cost is down to 8 rage points). Minor glyph of thunderclap(increases radius). Spec in all mob control and multi-target damage increase talents.
Now as for the fight..label your targets...any target(s) having a mana bar..is/are the primary target. Charge....thunderclap....walk backwards..make the mob come together forming a frontal cone target and shockwave.Rotate around them making them turn their backs to your group. Devastate..thunder clap..cleave..shockwave...etc. Its really not necessary to rotate targets. If one is getting away..use one of the taunts available to bring it back. Melee on target sequence hitting from behind..and ranged/healer doing AOE/heals from proper range distance(130% aggro).
You can add my two cents along with the other posts..there is no exact set best way..just pool the info your getting and you should have no more problems.
P.S. in my trash mob spec..I have piercing howl..You would be surprised how many times that has saved a run from a wipe..when an accidental patrol or additional mob pull occurs. Does just enough to control the situation and pull all the mobs into one place for a shockwave.
PSS. Know your mobs..some casters don't always move..which can cause some problems. Make sure they are targeted and charged first. Also a DK is good at pulling them in if necessary..or have them CC'ed if possible. Moving the whole party out of line of sight will also make them charge you.

I got a charge/shield block macro. Damage shield/thunderclap makes the mobs stick to me.
When shield block runs out, I use my first Shockwave. It will be 15 seconds into the fight before mobs have a chance to hit me. By then one or two will be dead.

I have no problems with threat, usually my engagement goes like this:
-Charge, pop bloodrage
-TC, SS, SW, Revenge+HS
-Switch Target, TC, SS, Revenge+HS
-Switch Target, SW, TC, SS, Revenge+HS
This is all dependent on cooldowns, but chuck into the mix shield block and devastate to refresh SS. Always maintain TC, and make sure once you've done your first 'pass' on the skull mob, to switch mobs as you'll have plenty of threat on that first mob.
The only two macros I use are:
-Revenge and Heroic Strike macro, that basically uses the Revenge icon, one button to click revenge, then click it again for HS.
-Focus Intervene. I always set focus as the healer, and use this macro so I can intervene if the healer starts getting smooshed.

With TC, Shockwave, and Cleave with Glyph of Cleaving you should be able to hold AoE threat through anything. If the newer tanks have any AoE threat issues, these 3 abilities should be all you should be using.
A SS and Revenge thrown on the first marked target should ensure a smooth run. Remember that cleave isn't on the GCD system, it uses your next attack. If you have the rage you can use it as soon as you get within melee range. Cleave is really the answer and I wish more warriors would use it. It makes it essentially monkey proof.

Every other class can aoe tank easy as and they don't need to watch their agro - need more agro and more aoe or perhaps change it so that our AoE damage stays and does damage over time

So not true. I have a harder time pulling aggro off a warrior than I do any other class. DK's take the longest time to get enough threat for me to start whacking things, paladins aren't that quick either - ok, once they have the threat they're unlikely to lose any of the mobs, but it takes time to get it.
Warriors don't really need to 'watch their aggro' any more than any other tank. On groups you mash all your "moar threat!' buttons, have a quick tab round to check your threat levels and then address any problem areas with a shield slam or revenge. Keep cleave queued up and you're done. Vigilance on the biggest melee dpser and nab yourself some extra threat for doing naff all.
OK, that's a bit oversimplified, but basically true.
On topic:
Not a lot I can really add to this. One thing I do (as has been mentioned) is to charge a group, TC when I get in the middle, and go straight through so they turn their backs on the dps. Now, if I've got some melee dpsers who I know are likely to cause me problems early in a fight I
won't
turn the mobs, just charge up and tank them as they are. Dps then have to go around giving me a precious second or two while they get positioned.
Once I'm comfortabe with my threat on the skull I'll have a quick tab round to check the others. I keep cleave queued up at all times (bloodrage and zerker rage for extra rage if necessary) and I save my revenge and shield slam for any mob I'm starting to lose threat on. Those two give you a hefty threat boost each, so good for increasing your lead on 'creepers' quickly.
After that I'll just randomly tab around a pop anything that procs/is off cd on things as I go. Obviously spam TC every time it's off cd. I like to keep my SW free just in case anything decides to run off - it's easier to stun everything to hold the escapee still while you re-target and wallop it.
