Gouge is useful for getting energy back while the mob is incapacitated.
Gouge your target, since it stops your attack you can gain some energy back while gaining a combo point. Get behind and Backstab away!

Depends. Its more useful in PvP than PvE. If you are going for a stun lock spec, then yes take it. Even if you are swords (which is a common stunlock spec), you can use it efficently.

Is it of any use for a combat sword spec? or am i just wasting my talent points in it?
Do not take improved gouge in a combat sword spec for the purpose of levelling, lightning reflexes is a better investment of the three points.

for a combat sword spec, gouge is mainly good for running away. Not worth putting talent points in for that though.

Gouge + bandage is an amazing tactic: it lets you stop the fight (presuming its 1v1) and bandage. This is useful both in PvP and PvE.
In PvP if you have improved gouge and 5/5 camoflauge you can usually gouge and restealth- one of my favorite combos is CS-->BS-->gouge-->restealth-->ambush-->5 point rupture-->vanish-->garotte.
You still use a backstab build for PVP?
