It looks good looks good, but since odds are you wont be having many diseases up all the time (and your Time on Target will be decreased) I'd advice you to skip BcB and max out Virulence instead (good if you're short on hit, and dispell resistance). Also Morbidity is very minor for frost, since you'd be using FS and DnD wont be used that frequently.
Btw, Anticipation is a tanking talent, try to avoid taking it.
You also ought to switch some points around to max out BotN, 15% on your hardest hitting ability is huge.
RPM and IIT are nice talents, but with both HB and CoI applying FF I dont see you using it enough to make a grand difference, and rpm is nice, but there's better talents further down.
Perhaps something a bit more like this would be an improvement?

(And the points in necrosis are shiftable of course)
I did include HC in that build because, well not every situation is a zerg situation so odds are it wont break that fast.
It's a great ability should you end up cornered and in need of a quick recovery/escape/bandage.

Acclimation is in my eyes far too situational and low procc chance (~30% per stack) to commonly be used in a pvp/pve build.
But should you desire to be a "bane of casters" and really have the tiny extra little advantage over them, it's possible to shift some points (perhaps from IFR or Merc. combat) and get it, I dont reccomend it however :)

Whilst a Frost/blood build might be better in terms of damage, on a pale horse and decreased DG cd is nothing to be ignored when it comes to pvp.

To each their own, but for disorganized bg pvp I'd rather just have the highest possible dmg. Roots/snares/slows are usually quickly reapplied, and are countered by Chillblains, CoI, and DG. If you can keep your rooter rooted, then you're even.
The exception might be for a Troll DK, who can gem an
Enigmatic Skyflare Diamond
, get 5/5 Toughness, and with
Voodoo Shuffle
have about a ~47% snare duration reduction.
I find Frost isn't so much about mobility and speed as it is about rooting and slowing. In mass disorganized bg battles, I just prefer to spread FF+Chillblains, keep it up with glyphed HB, then proceed to blow as many things up as possible b/f I get blown up.
I suppose it depends on gear though. If you've got enough crit and dmg, you can move those pts from the Blood to UH for more utility. You're just giving up +14% crit to Oblit and Blood Strike, +5% crit to everything else, and some additional AP, basically for OaPH, 10s off DG, longer lasting and stickier diseases.

If you're really just doing "disorganized bg pvp" then a full dps build with extra snares (like yours) would naturally be an attractive choice, the 0/54/17 build I linked can ofc be modified (i.e. 3 pts from frost to get Subversion etc, IFP is fairly weak) after your own liking.
But on the other hand, if you're really just doing "disorganized bg pvp" then why just not roll with a full pve build I'd say. Everyone has their own style I agree but I disagree with the "high dps first, pvp talents second" mindset, it generates some sort of hybrid build that's specialized at BG zergs...
For frost pvp I'm more leaing towards a 3/51/17~isch build, gonna have to try it out.

why are you going in unholy,? for OaPH? cause that just seems silly for 20% stun/fear reduction
I personally value virulence, Unholy command and OaPH higher than the early blood talents for pvp, the blood ones does offer better dps however, but I find the more pvp orientated talents in Unholy more attractive for pvp.

My Orc DK does miss OaPH. Stacking stun resist ftw, and even epic mounts feel like slugs without that 20% extra speed.
