I have found a decent way to deliver the Rend portion of the B&T rotation. I will start a multi-MoB pull by: Charge>SS>Shockwave>Rend. Then go into a standard AoE rotation keeping T-Clap on cooldown. By putting Rend up during the Stun of the SWave, I mask the low threat generated by the attack, while allowing the long term advantage of B&T to develop.
I mention this because I still hear/read Tanks complain about "having" to lead off with Rend. The sooner you can get the SWave off the sooner you can get a free Rend in.

Why would you charge and lead with Shield Slam on a 4+ mob group? Either charge and rend, TC, Shockwave, or charge, TC, Shockwave, Rend, SS, TC.

I would generally recommend charge-TC-SW-SS/revenge-rend-TC, with cleaves added as rage permits. Gets some frontloaded AoE and single target threat in, and then inserts rend before the second TC.

I start with the SS because, in a raid environment, the DPS will hit my main/1st target basically as I hit it. The SS gives me the most threat against the primary target. I SWave 2nd to control the rest of the pull, and to afford me a clear shot, early in the encounter, to land a safe Rend. I regularly can T-Clap just before, or right at the end of the stun and it's a standard AoE Tank job from there.
The idea is to mask the low threat Rend from the initial target while they are stunned and my threat is at a max. Give it a try. I find it very effective.

If they're grabbing the main target after 1 GCD, there's every chance the other mobs will be peeling after 1 GCD too if you don't open with an AoE. And losing aggro on 3 targets is a bigger problem than losing aggro on 1. Heck, if you lose one mob and taunt it back you've actually gained threat for no GCD or rage cost. In most such cases I would definitely go with the AoEs first, even if the DPSers aren't AoEing much. With TC and SW the mob should be stunned anyway, so not even positioning would likely be affected.

I currently do this for multiple mobs on my 70 Warrior:
1) Charge someone
2) Shockwave
3) Cleave+Rend (macro'd together)
4) Thunderclap
5) Tab while hitting SS/Revenge/Devastate, whichever is available
6) Cleave/TClap when off cooldown
I currently use three Macros:
1) Cast Rend+Cleave+Berserker Rage
2) Cast Shield Block+Shield Slam
3) Mouseover Taunt

I generally charge, thunderclap, demo shout then shockwave. Whilst cleaving if there's enough rage.
DPS should be allowing you to get threat now, not going 'HURDUR DPS' so threat shouldn't be an issue at all.
In cata heroics the DPS in groups _SHOULD_ be doing single target dps, so you're generally fine spamming cleave, tc and then filling with shield slams and revenges.
If you click the youtube link in my sig, you'll see a couple of heroic trash pulls in a couple of the videos; nothing major but you should see what I mean.

for now, in a raid environment, if dps starts during your first gcd, then they're doing it wrong...
better ask them to wait for a few more seconds before they start nuke everything down...
the more seconds allotted to you gaining more aggro lead on the mob, the better....
generally, SS, rend and Tclap, tab taunt next target, devastate, Tclap and shockwave... and if there is enough rage, cleaves too... keeping Tclap on CD makes sure your rend is always refresh on all mobs, especially when you spec for it....
