Lots of this stuff might sound dirty or gross to some people, but most of it is a non-ssue to me.
Where you say 1 long hair falls into a mixture and it gets pulled out. Hey, at least it got pulled out right.
If the food tastes good, I'll eat there. If the food taste good and I get sick, I'll try it again. If I get sick again, I probably won't go back. My deli by my old collage looks like a filth pit, but it makes the best Buffalo chicken sandwich I have ever had, I recommend it to everyone.
Cooking food gets rid of a lot of bad stuff. Not all the bad stuff mind you, but most of it.

Assuming you live in the US, if you think it's a major problem (and just about everything you listed is a health code violation), report it to the health inspector.
This
is a list of the states' Depts. of Public Health, which would be able to tell you how to go about reporting violations.
Edit: And this is why I prefer not to know what happens in the back of the house, because like hat if the food is good, it'll take more than one upset stomach to keep me away.

Close enough, the process is probably going to be very similar, although what constitutes a violation in the US may or may not be a violation in Canada, but it never hurts to raise suspicions, if you think something needs to be addressed.
Hopefully, Canada has whistleblower laws, but I'm sure they do.

You don't think someone putting their saliva covered finger into a pot of chili that is being sold is nasty? Or someone coughing right into a pot of food they are making? Would you really eat that?
If it tasted good, sure. If I got sick, probably wouldn't order again. Probably won't get sick from that though

When I worked in a restaurant, we weren't allowed to wear gloves or hats for some reason. The owners told us it looked tacky. But the girls had to have their hair tied back tightly and the boys had to have short hair. And we were constantly washing our hands. Make a burger? Wash your hands. Dished some fries? Wash you hands. Made a pizza? Wash you hands. I had the worst cuticle tears by the time I put my two weeks in.

At the place my dad worked, the workers used to have meals prepared for them. The chef threw a chicken on the ground before giving it to my dad and he got food poisoning.
That's not the worst thing that happened there, but I think some of it is going to tribunal so it's best if I don't reveal everything :p.

Fun fact: I worked at a deli for about a week until I walked out, disgusted and sickened. It took all the fun out of eating out seeing how it was all made. It was my first "Real" job, though I'd been a TA before for a quarter and a half prior.
Three weeks later I got a job as a community manager in the video game industry (job prior to Wowhead).
We had pretty good sanitary stuff, but I think the grossest thing I saw was just the sheer lack of experience. I had to make potato salads and more on day two of training, without even knowing the recipe. I am pretty sure I ruined many people's lunches that week >_< And I cut my finger once and kept making a sandwich, because it was my second time cutting m y finger, and I thought I might get fired since the first time the superior screamed at me and put me on closing duty for it. So that was also really gross and horrible, I felt bad, but also scared of being in trouble.
@OP: It sounds kind of gross, but not that bad. It was way worse than my deli though, so I do think it's substandard.(##RESPBREAK##)2##DELIM##Ashelia##DELIM##

Some of what you listed is wrong, some really isn't all that bad, and some it just the nasty truth about a commercial kitchen.
Food dropping on the ground, or even worse, into the garbage....yeah, I would say toss it out, or at least wash it off. First off , the loss of a single potato or piece of cheese isn't going to affect the bottom line of a budget, so even if its just on principle, it should be scrapped.
The issue with the hair and the finger tasting...Im not really to worried about that. Both items were being cooked, and even if there was a little germ transfer there, the cooking process would take of it. Finger tasting is really more unprofessional than unsanitary....use a spoon for God's sake.
The bleachy hands...I dunno about this one. From a sanitary standpoint, I am sure the bleach is probably 10x more powerful for sanitizing her hands, but I would want to wash them just to not risk ruining the flavor of the food.
Sadly though, I have worked in some very upscale, expensive resturants in the past, and in the back, they are justs as dirty as the scrubby diner on the corner. And the food inspectors are much more concerned with things like holding temps than they are with hair nets and clean dishes.

As a member of the food service industry I can say with total and complete honesty...Yeah, all non four star kitchens, especially grocery delis, are cesspools of filth. There is always one or two employees that care (for now, at least til their spirit gets crushed) about food safety. I will not eat at a rather large multi-national pizza chain because I was a cook for them for two and a half years. Fast food of any kind is disgusting if you know how and by who it was made.
Report them to the Health Inspectors is all I can say. Do it under the guise of a customer if you have to. And hope that the inspector isn't corrupt.
