For me, Classic would definitely be gnomer. I hatehatehate that place so much it's not even funny. BRD is long as hell, but it's still really fun imo.
BC...there really isn't a 5man in BC I dislike very strongly that I can think of. But since I don't have one I dislike, my favorite is definitely Old Hillsbrad Foothills.
Northrend...I don't really like either of the Ulduar 5mans, or OK, but I despise the Oculus. Hands down.

I love love love Halls of Reflection. I just wanted to say that, before everyone comes in saying they hate it.
None of the LK five mans are that bad, a few are glitchy, and a few are boring. Nexus, HoS, HoL probably all share that last aspect.

Classic: Wailing Caverns. It is very long, I always get lost (yet I can find my way through Gnomer just fine), and it seems every pull is the same. When running lowbies through, mobs had this annoying pattern of tethering on me and running back to kill the lowbie (no idea how this happened, because in instances it's not supposed to), which made it a pain.
TBC: I barely did heroics. Kara was easier and dropped better gear. The instance itself would probably be mana tombs, simply because the group would never stay for me to do my quest.
WotLK: HoR. I have no problem with a fight that's difficult. I have a problem when every time on wave 4 I get CCed for 3 seconds, and during that time I go from full to dead.

Classic; Blackrock depths, just because of how long it takes.
Burning crusade; Arcatraz, not sure why, i just dont like it ^_^
Lich king; Halls of Reflection, i HATE that place. Tanked it around 20 times for a guildmate, and the offhand never dropped for her, then we went to icc10 once, and the one from deathwhisper dropped first time! D:
Overall; i hate HoR the most :)

Vanilla - It has to be BFD, its just so ridiculously boring. Apart from that, I simply love EVERY dungeon in vanilla. Level 40-60 ones is awesome <3 ^^
TBC - All is kinda nice actually
WoTLK - Occulus because its just boring.
When it comes to favorites BRD and ST(armill3, its an awesome instance) flies on top of vanilla, Shadow lab(Dieckmann, its an awesome instance because its actually a bit hard) in TBC and in WoTLK every instance suck imo.

Classic: Uldaman hands down. I always seem to get lost in here.
TBC: The Mechanar was the hardest for me to solo for some reason
WotLK: Occulus hands down. Just so much that can go wrong.

Depending on the group you have HoR can be a total faceroll or a wipefest. I like and hate the place at the same time. As much as I extend the ID for that instance every single day not to get into it via random dungeon finder, I always agree to run it with my guild because it's fun with people who have a clue.
In TBC actually my favourite instances would be Shadow Labs/Arcatraz/MT... Nowhere as difficult as some try to paint them, I remember that one time when we went into Shadow Labyrinth some months into TBC when everyone had their T7-ies, facerolled through the instance in 30 minutes and at the end the holy pally asked "Wait, you sure that was heroic mode?" and then someone replied "Yup, I just looted a badge". Huge pulls, lots of CC, lots of kiting, lots of fun (TIME FOR FUN!).
The one I hated the most in BC were Crypts. I never got my groups to understand they had to DPS possessors first. That was also my first heroic and we made it to the 1st boss with a tank full decked in blues, two arms warriors and only one mage. That was epic, but not in a very enjoyable sense of it.
