Mantle of Shadowy Embrace
My druid tanking shoulders were horrible so i started farmed for these and after maybe 2-3 runs they drop and a resto druid from my former guild (glad I'm not in the same guild as him anymore) needs them and wins, he just says "Sorry man but i need them too". Ofcourse it takes like 10 runs more for me to finally get them.

When we just start our Kalazhan, drop
Bands of Indwelling
Pri : need!
Me: You have another good epic healing wrist, would you like to pass it? Others pri only have blue.
Pri: It's better then mine!
She have
Bands of the Benevolent
. And it happen almost every time we go Kalazhan till she /gquit . Thanks God...
Sure, perhaps it was a little selfish, but an upgrade is an upgrade. Unless you cover this sort of situation in your guild loot rules, upgrade = need, regardless of the degree of the upgrade.
That being said, I would have asked the person to pass as well. However, I'd also probably have rules somewhere indicating that passing is expected in that instance. :)

It was on my Shaman, so I didn't want the sword, but damn, it pissed me off so badly... can't even imagine how the warrior/rogue were feeling xD.
I had a warlock roll need against my mage for
Incanter's Cowl
. basically, he said he rolled because he saw the spell damage was better, neglecting to read the rest of the tooltip.
I let it go because it was a guildy, but still. Reading is fundamental!
Its not even about reading, its just knowing the sets. If I know a certain player needs an item that's a set that's for their class, I'll pass even if its an upgrade to me.
Ideally, that's how a group operates. :)

I think hunters are the most extreme example of any class. There are a ton of really dumb and annoying hunters out there, but man--when you get a good one, he is
awesome
. I have some friends who regularly run heroic Shattered Halls without a tank--just using two BM hunters, literally holding aggro on all the mobs by rotating Growls on different targets, all while DPS-ing full time. It's amazing.
