My suggestion would be to scale everything down and make your UI as minimal as possible - no fancy artwork and only essential information on the screen.
Don't show any action bars for which you use keybindings.
Hide combat logs/dps meters/etc. so they don't take up space when you don't need them.
Keep grid small and don't put a background on it.
Pitbull and agUnitFrames can both be configured to be very minimal (Xperl too, for that matter - scale down, remove the portrait and big borders and there you have it).
You can also take a look at some of the minimal UIs here or at wowinterface for inspiration.

I've only got a 15" monitor myself; the first thing I did while making my current UI was go into the video options, and check the UI Scale option, I ended up dragging it to about halfway, which for me was the balance between readable and small.
After that, Pitbull (3 is a complete version, but a huge effort in setting up, 4 is a beta version, but is much simpler to set up) for the UI, and Macaroon or Bartender4 for the actionbars. I don't use Grid myself, so I can't give any help there, I'm afraid.
