You're right, it is an addon. There are so many addons out there that can change the default UI graphics in WoW. It's crazy that someone would assume someone went into the game files some other way and changed the graphics. That can get you banned by the way.
If you or the OP took the time to Google the guides I mentioned, you'd see that's not true. You can replace virtual files the game uses by mimicking their path in the WoW folder. The game files don't actually need to be modified at all to replace the core files, and it's perfectly legal.
This exact texture is replaced often in this fashion.

I'll have to find it again. It's a pretty old article and I can't remember the name of it. But I recognized it because the texture those buttons were changed to is the texture originally used by the guide, and the work looks identical.

The download provided by the OP did indeed contain replacement textures in the fashion I explained. The game packages its files in archives, like Zip files, and at load times it unpacks them and places them at virtual paths. By placing your own textures at these paths, the game uses them instead of the unpacked files. You can exploit this to replace files without actually replacing the files. And the game is hardcoded to only allow it on certain file types (IIRC), so it's considered legal.

. Open the Interface subfolder, and the three folders present besides the AddOns folder (Buttons, DialogFrame, Minimap) contain what you need. Place them within your own Interface folder to replace the art. The files are named pretty logically if you want to remove any of them.

The OP wasn't looking for that. He wanted to change the buttons in the inventory pane, not the inventory pane itself.
