I know we shouldnt trust tooltips explicitly. But looking carefully they say
Meditation "allows" 50% mana regen while casting, which means that if no other effect goes on, then you will have 50% of your out of casting regen.
Spirit tap has two effects.
First says that it increases your spirit by 100%, and while there are other factors into out of casting regen, lets assume that means that the number that meditation looks at is doubled, thus helping meditation.
Then it states that your mana regeneration "will regenerate at 83%" Which
sounds
like it would overwrite any other mana regeneration effects. however the same wording "will regenerate at 33% percent" is used for improved spirit tap. That would suck to have regular spirit tap, then get a crit and have the regen effect nerfed.....
So what likely happens is that both spirit tap and improved spirit tap add mana regeneration EFFECTS onto your character. This is backed by the idea that you see the buff's whenever they proc.
However you never see the meditaion buff, so to my understanding and hope, All three stack.
EX:
out of casting Mp5: 100
in casting mp5: 0
In casting with meditation: 50
Out of casting under Spirit tap: 200
in casting under spirit tap AND meditation: 100+166=266
out of casting under spirt tap AND improved: 220
in casting under spirit tap AND improved AND meditation: 110+182.6+72.6=365.2
Now this does mean you could regenerate more while casting than not casting....Do i see issues with this no. Because you are generally casting spells that cost more than 365 every five seconds if you have 100 mp5, thus it looks like an incentive to keep casting....but you are using mana while casting....
sadly if they do not stack the last lines of the three conditions would read 50, 166, 72.6(or 182.6 depending on the order of procing) respectively.
Again I acknowledge that intellect mp5 gear also effect all of this....but to see it in a manner that is understandable, i made some assumtions. so please do not think that if you have 100 mp5 on your character sheet that you would have 365 if all three are up
MP5 = 5 * (0.001 + sqrt(Int) * Spirit * Base_Regen) * 0.60 rounded up
where "base regen" for lvl 80 is 0.005575 from wowwiki.com . . . .
see why i wanted to simplify :P

Your in combat mp5 can never be higher then your out of combat mp5.
Say you regen 100 mp5 out of combat unbuffed.
Meditation allows you to regen %50 of that all the time.
Now Spirit Tap doubles your spirit so you will instead be regening 200 mp5 out of combat
Spirit tap also gives 83% of mana regen while in combat so even without meditation you would regen 166mp5.
Now you add in meditation and you will be regen 200 mp5 in combat since your in combat mp5 can never be higher then you out of combat regen.
Also improved spirit tap and regular spirit tap do not stack if regular spirit tap procs while improved spirit tap is active it gets overwrited since regular spirit tap is a stronger buff. (In wow if you have 2 buffs that have the same effect such as a stam buff scroll and a priests stam buff the stronger buff will overwrite the weak one. This can be annoying since wow only looks at the magnitude of the buff and not the duration. So if you suddenly got a +1000 stam for 10 seconds buff it would overwrite you Fort buff)
