Same as most people - I'd love to live forever if it meant eternal youth. Think of all the amazing stuff you could get done! The option to die if I ever get bored of life at, I dunno, age 12,000 or something would be nice.

Honestly this is a tricky question....too many variables. if i could stay young, had the option to die, and my body had some sort of regenerative abilities so that i could stave off diseases, illnesses or other crippling situations. Imagine you get hit by a car or something and your legs get amputated. would you really want to live forever with no legs?

If it's á la elves, then yes. I probably would. I guess it depends if other people got that opportunity as well (I'd be more inclined to want to live for millenia if I didn't have to watch everyone I know and care about wither and die) , but I think I would regardless. I'd definitely read everything there was to read and enlighten myself as much as possible. Then top myself when I decided that enough was enough.

Sure, but with long pauses (something like winter sleep).
That's a cool idea. Since if you can live forever, you presumably don't have to eat. So you could easily sleep for years at a time if you chose to.

This would help if someone decided to bury you in a concrete mold into the deepest chasm of the deepest ocean. I like the option of a physical death, in the event that you get accidentally buried alive.

If I can still die from significant damage - yes. If I can stay somewhat young (and by that I mean don't go all Face of Boe-y), yes. Without those two, no - I would not want to be an immortal person that will age and age and age until I'm like parchment and break from the slightest touch, and yet cannot die. That'd be utterly terrifying.
I mean, there are so many things I want to get done, and I know I won't be able to do all of them because quite frankly, many of them are too time-demanding. I'd like to see where the human race ends up, and I'd like to see us reach space. And also aliens.
All of those, I believe, will eventually come along - I'd like being around to witness them.
