It's probably not nearly the worst, but it's bad either way.
In Blackrock caverns, every time i killed a boss, i thought I was supposed to run back to the start and hand in the quest. I had no idea the quest popped up when necessary. Yeah, I wasted a lot of people's time.

I've got two 'mistakes' that are pretty hilarious. They're actually three but two of them goes together.
First out, a Blood Elf Rogue was my first toon, on a starter account about three months ago. Had no idea that there was a portal in Silvermoon to Undercity, so. at lvl 9, I skip Ghostlands and run, yes run, through all of Easter Plaguelands. Because I wanted to get to the Zeppelin and go to Orgrimmar, which was the only thing I was slightly familiar with from playing a couple of times in Vanilla.
Once in UC, flying in from Brill ... Let's just say it took me several days before I gave up and flied out again, I had no idea there was elevators. Same thing in Orgrimmar. I fell from the plateu where the Zeppelintower is, died, rezzed, and had no hell of an idea how to get up again. Ended up deleting that toon because I thought I would never get her out of there...
Oh, and on my Hunter. At lvl 10, left my pet in Silvermoon to go and tame another Dragonhawk. A bigger one. Fuu I was pissed when every single dragonhawk I tamed shrank when it came to me ... Must've tamed almost every single hawk before realizing 'Okay they're not going to remain big' ...

I rolled Alliance. I didn't know what I was doing, you can't hold it against me!
Honestly though, I had no idea how to use the AH. I was level 18 before I made my first gold. This was in Vanilla, and let's just say playing a Mage while not having a clue, on dial-up, was quite painful. Having gold would have made the situation considerably easier. My Mage did not survive long enough to see TBC.

Recently? I'd been PvPing. So I had my holy set on.
Got out of BG's and went in to tank for my guild on a few heroic DS fights. First one was Morchokk.
Let's just say I went splat.

Enchanted two str 1-handers with icy and fire on my hunter because they looked awesome :P
at level 30 XD
farming turtles in hinterlands because thottbot told me that they had the highest drop chance for hurricane (epic bow)... instead I walked out with like 20 stacks of turtle scales that got me the money for my epic mount :D

I Mass Summoned my entire 25-man raid once to their deaths because I did it too close to the edge and they all fell off when they accepted. I'm not ashamed or embarrassed. I laugh lots.
LMAO that's a good one. =)
Let's see...dumbnest/noob thing I have ever done?
Awe, when I first started playing I made a shadowpriest and tried to heal in that form...let's just say /kick =)

My first toon ever was a mage. This was back in Vanilla when the paper doll had an ammo slot for ranged weapons, regardless of class. After equiping a wand, I purchased a bunch of arrows to use as ammo for the wand. Around level 20 or so, someone had inspected me in Orgrimmar and whispered me that ammo was useless for a wand.

first few minutes of playing in durotar, I saw people talking in /1 so I kept trying to talk with them... eventually my friend over my shoulder saw I was using /s. guess I shouldve read the manual
I was level 60 in vanilla before I realized you didn't have to click the spells :X years later I still can't get used to it

Before i played WoW i played korean MMO's and games like lineage 2 and diablo 2. Every game has a trade window like WoW except for the fact that if your just giving your friend items it's faster to just drop items on the ground.
So when me and my friends first started playing the game we had no clue that if you dropped items outside your bag it destroyed them. We stood there for like 10 minutes going,
"where the hell did it go?"
"Did you drop it?"
"yeah it should be right there next to me"
"ummm no, there's nothing there"
"well i dropped it did you pick it up already? is it in your bags?"
"no it's not in my bags"
"well i dropped it cause it's not in my bags"
"i think it disappeared....i don't think you can drop stuff on the ground"
i finally dropped one more item just to test and we sat there laughing about it and feeling pretty stupid. It kind of sucked that we were just starting out with no money or bags and i probably lost like a good 5g in items and my friend didn't get the equipment or bags lol.

My first dungeon... I kept rolling need until I was booted.
I ask one of the members why and he said..."you were being greedy."
I responded back "But I wasn't clicking greed, I was clicking need."
A little pause...where he was probably laughing his ass off and telling the other members and then I got re-invited back to the group.
They explained the roll mechanic to me. To which I was grateful and embarrassed.
We enjoyed the rest of the Deadmines together and I received a well deserved ribbing along the way.
My first friends list members.
Pre-LFD TBC days, when players were willing to be a little more forgiving.
