Some things I'd recommend for playing as a medic:
Stick to a heavy, soldier or some other class that can do a lot of damage (like pyro). You'll keep them alive, allow pushes to take tightly held areas, and generally be a credit to the team (heh).
LoS, always. Make sure you can keep your target constantly healed, but hide behind corners and poles and the like so you don't draw fire. Rule #1 of any game is to kill the healer, so you'll be target #1.
Spy check anyone that comes near. Use your syringe gun or saw to attack. If the attack does damage, it's a spy,, and you should kill it.
Beware of the Dead Ringer! If you kill a spy in one hit, be prepared for that spy to reappear somewhere nearby. Listen for the *bzzzscht* sound that the Dead Ringer makes when a spy uncloaks.
Use voice commands (z or x bring up the main panels, then you can select commands from there).
Know when to use an ubercharge, which makes you and your target invincible (unless you're using another medigun like the Kritz Krieg or the Quick Fix). It's better to use one too early than too late, but be prepared to use one, especially if you or your target is about to die. Also use them if there are a lot of enemies near the capture point or if there are lots of sentry guns, etc.

Spies can do both but it takes some attention to root them out. Once you have it down, they'll be glaringly obvious to spot.
As a Heavy, you have the highest dps output of the classes. A good but slightly expensive way to spycheck is your Minigun.
Being the slowest class, you are an easy target for a Spy. Use your minigun on teammates for a few seconds, if they're a Spy the damage will kill them.
Things to note about spies:
-Being struck or colliding with an enemy will cause a cloaked (invisible) spy to "blink." Meaning that an outline of him will show for a small duration of time.
-Even though they are disguised, they still cannot walk through you like your teammates can. If you unexpectedly hit an object you expect to go through, shoot it the @#$% up.
-There's a device a spy can use to fake his own death. The price for that is a loud decloaking sound. If you hear it, be ready.
A good habit to pick up as a Heavy is to jump around corners and rev up your minigun in midair. You maintain your speed without the slowing penalty of a revved minigun. Trust me, it helps.

Soldiers are the best class to stick with (obv.). They keep scouts off you, have good mobility and can damage themselves to build your uber faster. Demos are important to keep buffed due to sticky jumping costing a large chunk of health. Pyros are fun sometimes, but the problem is that they require close range, leaving you very vulnerable.
Just to build on this point, medic/demo survival is often the most important thing. If you're healing a soldier and you see an ubered combo run in, start running. Leave the soldier to die (preferably delay them). Uber takes a fair amount of time to build and downtime without a healer leaves your team at a severe disadvantage.
I don't think you can tell when you're damaging a disguised spy.
Seems about right. (can't skip numbers in an OL >.>)
Better yet, use your voice. Plug in a mic and yell at the team, that's what medic's do after all.
Knowing how to act while ubered is also important, do a slightly lesser degree. If you're ubered, you don't want to lag behind and cause your uber to drop off. And I find that most of the time, running in front of your target is the best thing to do. It blocks knockbacks (sentries x_x) and allows your target to run freely ahead.
Oh, and, make sure your target has ammo =.=

I don't think you can tell when you're damaging a disguised spy.
It makes blood spatters.

You can't. Not even knockback from rockets or stickies will affect a Spy.
I'm not sure about from a Pyro, though. In my defense, I've never tried as 3 ammo spent on a spycheck is cheaper than 20 on a backblast.

Also, I just managed to build an engy nest on 2Fort in the enemy base (where their intel is) TWO MATCHES IN A ROW lol.
Needless to say we won both 3-0.

You can't. Not even knockback from rockets or stickies will affect a Spy.
Blood spatters, flames, bleeds all show spies.
Also, turn on the option that makes a ding noise every time you hit an enemy. Shooting a disguised spy will cause the ding to go off.
