Yeah, when I said asap - I ment when the player got behind the add. Where are we going wrong to still have 3 adds up at the end of phase one?
What adds are talking about? There are 3 possible adds, and they are all very different.
Starting the transition, your off tank will have 1-2 Horrors on him and some ghouls (3+ usually) and a plague bouncing around them. This is OK because about 30 seconds into the transition, they should all simply die to the plague.
During the transition, new adds will spawn (Raging Spirits). Those have to be single targeted and killed by all your dps (except the one killing the frozen orbs) immediately.
If you have 3 raging spirit adds up by the end of the transition, that is very bad. Your DPS could be going afk :P You should aim to have it down to 1 raging spirit at the end, and kill it before the 1st Valk arrives.

The way to deal with the plague is very simple. Just follow the few rules:
1. Off tank must be separated from the raid
2. There must always be an add on the off tank
3. Every single plague must be dispeled in a manner that it jumps on the off tanks adds
4. No one DPS the off tanks adds
Thus, what you want to do is to have all the ranged in one spot, all the melee in another spot (on the LK) and then there is the off tanks spot.
Each time there is someone infected with the plague, he alone runs in front of the off tank, right behind the adds, where the plague is dispeled, and then returns.
This way there will never be more than two shambling horrors on the off tank, and one of them will be already almost dead, with last ticks of the plague killing him. As long as there is any add on the off tank, the plague will remain there, growing in power.
During transition phase, the off tank must remain separated from the raid until all the horrors/ghouls die from the plague (its very fast now, the plague is stacked high).
The plague will then jump on the off tank and has to be dispeled - only then is he allowed to get closer to other raid members.
To sum it up, just follow the 1.-4.
