The macro conditional system is the
only
means by which a macro can choose a spell to cast, and
it does not include any condition based on position or facing
- intentionally. Even a Lua script running from a macro cannot ask about your target's facing, and even though it might be able to make reasonable guesses about that (at this point we're talking about code on the scale of a small addon, not a macro), such information
cannot
be used to choose a spell.

.
And, for the record, my main is a feral druid - I know the frustration you're talking about. The only solution is to learn to judge your latency and develop proficiency at quickly assessing your target's facing based on partial information (what you can see in the middle of the melee cluster****).

Well, you've jumped right in the middle of the minefield, havent you?
Your best bet would be something like this:
#showtooltip Shred
/cast Shred;MangleWhat this will do is ONLY show the tooltip for Shred...when you can use it(I.E You are behind the target), the tooltip will lighten up. When it does so, hold down alt or click with the right mouse button to use it. If you just spam the key it will ONLY mangle. You can then SEE when to use it, but you still need to decide yourself when to use it.

As a person who plays a feral DPS as his main, I would recommend that you just get used to finding out and remembering what side of everything is the "back". I wouldn't want to have to use a modifier to choose Shred or Mangle with them both bound to the same button.
In PvP ... if you have any kind of latency and if your target has any kind of latency the chances of you landing a hit from behind them are slimmer and slimmer as both latencies go up.
Like they said, you have to make the choice for yourself and sometimes it's a better use of your time and energy (literally, energy, the resource) to just hit Mangle instead of spending
n
seconds doing nothing but white attacking trying to find something's back.
