You can macro envenom on the same key as evis, just with a modifier added.
/showtooltip
/cast envenom; eviscerate
You can use ctrl or shift instead of alt if you prefer.

Can someone help me with a macro? I want to press this one button and only this button.
HALP!
Priority like this:
1) Kick if target is casting
2) Gouge if Kick is on cooldown and target is casting
3) Kidney Shot if Kick and Gouge are on cooldown and you have 5 combo points and target is casting
4) Envenom if I have 5 combo points and they are a plate wearer
5) Eviscerate if I have 5 combo points and they are not plate
6) Mutilate if less than 5 combo points and not casting
