There are a few people who know him as Ner'zhul: Kel'Thuzad, Mal'Ganis, and Kil'jaeden to name a few. The reason everyone calls him the Lich King or Arthas, is because that's what they know him as. The Lich King is the true name for the being in question. It is basically the merging of souls of Arthas and Ner'zhul. Technically there is no Arthas or Ner'zhul anymore, but only the Lich King.
As far as his comment, he's the Lich King, he can say just about anything he wants.

RIght, but my question is how many people outside the Burning Legion, the Scourge, and the Cult of the Damned actually knew that it was Ner'zhul at some point? Yes he can say that he was a shaman since Ner'zhul was one, but how many people would understand why he said it?
Well, I would assume not very many... but then there aren't too many of those who knew him as a Paladin either, and only know of him as the Lich King. My guess would be that some of the members of the Old Horde may know that the Lich King is part Ner'zhul, but that is doubtful. So... Medivh, maybe?

Ner'zhul came to be known as The Lich King long before he merged with Arthas, and I'm quite sure all of his servants knew who he was. Remember the Acolytes from Warcraft III: "My life for Ner'Zhul!". Pretty much all the Scourge knew, and thus - it shouldn't be hard for the rest of the world to know it from that.

It's actually simple to say it now: There is no Ner'zhul, there is no Arthas, there is only the Lich King. Yes, most people say Arthas, but I'm sure there are a large number who say Ner'zhul instead. It's a moot point now that both of them are merged into the King of Undeath, the Lich King.

No, a creature wearing the Lich King armour with Frostmourne kills the Souls of Arthas(mattias) and Ner'zhul, leaving only the undeath. It's Arthas's body and experiences with Ner'zhul's memories and powers given to him by Kil'jaeden. As been said on other threads: The Lich King can say all these things and they will be true: I was a shaman, I was a Paladin, I was a Warlock, I was a Knight, I was a Prince, I was a Clan leader, I was an Orc, I was a human. All of that doesn't matter anymore, there is only the Lich King.
