For me, its between 4 that have been mentioned a few times already, but each has a different reason for being difficult:
The Oculus.
This instance is quite hard (particularly the first and final bosses) the first few times you do it, and remains difficult basically right up until you're able to master those fights. Once you and your group have them well rehearsed though, its quite an easy (albeit monotonous) instance. The difficulty of this instance has very little to do with gear, and much more to do with player skill and knowledge.
Halls of Stone.
Quite a gear-dependant
and
skill-dependant heroic. Krystallus and Tribunal of Ages in particular are difficult encounters. A skilled tank is especially essential for the Tribunal encounter.
Halls of Lightning.
This instance is an interesting one, in that its more
class-dependant
than anything else. The trash mobs require some surgical pulling by the tank, and having an all-ranged group helps. The Loken fight happens much more smoothly if your healing is of the raid/group healing variety (holy priest, shaman, druid). If everybody can stack on the tank, even a blue/purple "heroic" geared healer can get the group through it, without anybody needing to dodge the nova.
Ahn Kahet: The Old Kingdom.
Like The Oculus, this instance is more about skill and knowledge than it is about gear. I've been in naxx 25 geared groups that fail just as hard as a group with much less gear. The healer needs to know how to avoid being silenced by the trash on the two platforms, the tank needs to know how to pull casters using LoS, and everyone needs to know how to avoid Flamestrike, as well as how to stay alive while being attacked by 4 non-elite mobs (Herald Volazj encounter).

And Gonglez does have a point, how can you level up 79 times without figuring out how to play your role/class?
EXCEPTION. Death knights, and people wonder why they are idiots some times, its because they got 5 levels to figure out how to tank ramps while other classes get 60 to figure out what they are doing. Cut us some slack, i admit i was one of the noobs, but im currently progressing thru ulduar so not all dks are bad. yes that was off topic, but i felt i should say it the way i talked myself into a corner and am too lazy to delete some text.
Stop. Just stop. Making someone level from 1-80 doesn't mean they get more tanking experience. The only way you can get actual tanking experience is from tanking, for
groups
(none of that solo, pull everything and kill it bull#$%^). And you can level from 1-80 without setting foot in an instance. Starting at 55 doesn't make a lick of difference to your skill or ability.

UP.
on heroic is very hard to do the drake-rider without getting killed.
his whirlwind can cause a wipe quite fast when not countered fast, plus the whole mini gauntlet adds to the dificulty.
as far as instances goes the drake-rider on UP is the hardest for me due to the fact taht rarely I doit without a wipe or two.
the other bosses can actually be done without wipes with enough coordination.
as for the oculus :S I've never been able to do this one, plp just never wanna come to that one, I've given up trying to get a group there.

Honestly, there is no "hard"heroic, and Gonglez seems to have the same opinion as my older brother who scoffs when im stressed doing heroic Jedoga or Trib. So i decided to watch a video of H MGT. I never once looked back at the heroics im doing as "hard"again.
H MGT was hard? All I remember was 1 paladin tank made most of the pulls ridiculously easy. The only "hard" boss in that fight was the gimmick 5v5 Free for all battle with no aggro table.

This is how a conversation would go between 5 people with the best gear in the game about the daily herioc pre 3.0
Tank- "Whats the daily heroic today guys?"
Heals- "Heroic Magister's Terrace"
Tank-"Eff that, I'm still recovering from my repair bill from Sunwell. I actually wanna make money this week. My epic flyer isn't going to buy itself! Gawd I'm bad!"
DPS Warlock-"Well if we could get to Delrissa at least I could get a really nifty trinket"
Tank- "Well we got a bad setup though, only two people with us can CC, and we need CC because we're BAWD."
DPS that couldn't CC- "Sigh... I never get invited to anything because I have a small brain... If only instances didn't require thinking!"
Fix't
I've did H MgT back in the day with zero CC. People were just bad and made up for their badness by stacking the group with CC. Yes, I beat the melee group on heroic with no CC because I actually thought through what was needed to beat them.

Nothing in guildruns.
Only time i ever found OK hard was the first time to learn the instance. But tbh it really isn't hard for me at all as a DK tank.

Azjol Nerub. The trinket always drops, some moron (dk, ret, hunter, rogue or lock) always needs on it and wins, getting the tank angry. Party disbands. Have anyone seen the last boss of that place?
