I would try and get
Trinket
from heroic AN to go along with your Valor medal. If you can get the PvP helm, it will be a great upgrade since you can whack a
meta
gem in there too.
As for gems, I would gem 8 agi/ 12 stam for the red slots and 24 stam for the blues. Gemming for pure dodge is less than optimal.
As for defence rating, it is unnecessary if you have 3/3 SoTF. Although you will find it on some gear such as rings and necks that are still good pieces to use.
If you are having trouble with threat, it's likely to be because of your low hit rating (0.15%). 5% hit and 10% expertise are good numbers to aim for to improve your threat generation.

Also one of the reasons that you are haveing trouble holding threat is your hit rating... while expertise is more important for a tank a hit rating of 4 is terrible.
