Could someone explain how [target=focus] works and how best to use it?

The example I always give is a mage in an instance that requires crowd control. If the mage has this macro:
/cast Sheep
he can select his sheep target before the pull and type /focus. He can then target Skull (first kill target), and by hitting the macro he'll sheep his focus without actually switching targets to it. That way he can attack skull, attack the second target, attack the third target, and whenever the sheep breaks he can hit the macro to re-sheep without physically selecting the sheep target.
Warriors can do this with Vigilance if they want. At the start of a raid he can Focus his vigilance target, and throughout the run he can just hit that macro instead of going through the Raid Frames and actually targeting the DPSer. This type of thing can also be useful for Intervene. Focus your healer and you can intervene the healer with one button press without changing targets.

Small example of how you could use focus (I dont heal myself so im just giving an example):
In an instance /focus on the tank
Replace all of your healing spells with macros that cast healing spells like normal BUT when you rightclick them, so you can heal the tank without deselecting your target

Basically, /cast will cast whatever spell you choose on the focused target. If you try to cast a healing-spell on a hostile target, you will fail.
Personally, I use on all my healingspells. I just hover my mouse over the raid frames and BAM, they get healed.
