Losing agg on patch

Ok here is situation. I MT Patchwerk, dk with bigger healthpool eats hurtfuls. And with irregular intervals i get hit by hurtful strikes, meaning im not 1st on threath like i should be.
I use shieldslam>revenge>concussion blow/ shockwave>devastate. Every auto-swing i queue heroic strike, using a 1,5speed weapon.
I also use retaliation, switch to battle-retaliation-switch back to def when i got some threath to get even more threath.
I have a deep wounds build (I OT more often than i MT so i like the extra damage and treath)
The schmenismeterz shows that i am doing about 2,2k dps, which i believe is very high for a 10man tank to do on patch, often doing more dps than some of the dpsers. Dk who tanks hurtfuls is at about 1400 dps, thats like 800 dps less, yet i get outthreathed from time to time during the encounter.
So, why? anything wrong with my rotation? The dk has imba rotation? Dks frost presence and abilities have greater threath modifiers?

patch hits the person with the highest health with hatefulls. its most likely that your healers aren't healing your OT fast enough between hatefulls and thats why you're getting hit with the ability.
This is incorrect.
10 Man Patchwerk, the person with the most threat (main tank) can not get hit by Hateful Strikes. The person with the second most threat WILL get hit with hateful strike. It doesn't matter if the 2nd person has 40k, 20k, or 1k health; the person that is second on the agro table (and ONLY the second person on the agro table) will take the hateful strike.
25 Man Patchwerk, the person with the most threat (main tank) can not get hit by Hateful Strikes. Hateful strikes will hit either the second or third highest on the agro table, whichever one has more health. Hateful strikes will ONLY hit the persons who are second or third on the agro table (excluding ranged DPS of course). The Second or Third person, whoever has more health is the one who takes the Hateful Strike.
As to the original poster, if the situation is as you have described it, the only solution is either for you to do more threat or for the OT to hold back. Your rotation looks fine. Your description of the DPS seems like you should be fine maintaining top threat. Either 1) you are mistaken and you aren't really taking any hatefuls, or 2) ? I don't know...

Hateful strikes also add threat to the tank that it lands on. A good portion of the threat the OT produces is generated from the hateful strike. This is from Patch's description:
Hateful Strike - Patchwerk will use this ability every 1 second, hitting the person with highest health in melee range for 27,750 to 32,250 (Heroic: 79,000 to 81,000) physical damage, mitigated by armor. As a physical attack, Hateful Strike can be dodged, parried, or blocked. This ability also adds threat to the top 2 (Heroic: 3) people in Patchwerk's threat table.
Essentially the OT DK can probably just go afk after attaining 2nd place in threat. Since he's usually the MT, he's probably use to using his highest threat rotation and abilities. I would suggest he actually slow down his threat and not use some of his high threat abilities so he can maintain 2nd place on the threat meters.
The other option is to raise your threat. You didn't link an armory, what's your hit rating and expertise rating btw?

In my "Patch-gear" its 6% hit and 5+% something expertise , i think i have a wierd weapon on in armory. Switch out that wep for slayer of the lifeless, enchanted with +20str and the dodge trinket for shard of contempt (a old trinket from TBC with expertise on)

your expertise is a tad low, that might be contributing a bit. at 6% hit, the weapon chain will do more for your threat that the strength enchant.
vigilance on the OT is a great idea if he is not having problems with the melee pushing him on threat.
your attack priority sounds good the way you write it, but do you dump a combatlog and analyze it after? WWS or wowmetersonline are great tools for figuring stuff like this out. if you can post a link to the fight, someone might be able to spot something for you.

For single target threat:
1. Shield Slam
GCD
2. Revenge if available (+HS) - else devastate.
GCD
3. Return to 1 if SNB procced - else Revenge if available (+HS) - else devastate
GCD
4. Return to 1 if SNB procced - (refresh TC if needed for survivability) - else CB / SW if available (you can do this here since a SNB proc doesn't make any difference. SS will come off CD anyway) - else Revenge if available (+HS) - else devastate
GCD
Return to 1
Fill in as many HS you can without getting rage starved.
Blow Shield Block and block value trinkets whenever ready together with your Shield Slams (macro it).
Put Vigilance on OT if he can still stay second on threat - else on highest threat DPS'er.
