This Endgame Gear Guide presents the Best in Slot gear choices, gems, and consumables for the Poison Summoner Druid build in Diablo 4. We recommend the most effective Legendary Aspects and Unique items for each slot with options if these are unavailable, Gems to use in socketed gear, and general-purpose Elixirs and Consumables.
Poison Summoner Druid Best in Slot Gearing for Diablo 4 Season 4
This setup represents the ideal gear combination you can get for the Poison Summoner Druid. Since many of these items and aspects are based on drops, this may not be possible to achieve. Alternatives are presented in the sections below.
For Legendary Aspects, ideally you'll want to replace these on top of other Legendaries with undesirable Aspects, since Legendaries will have 4 affixes instead of 3. Rare items with very strong Affixes, particularly Greater Affixes, may be a good choice to receive Legendary Aspects, even though Rares only have 3 slots.

This is an awesome aspect for significantly improving the damage potential of , as the skill’s cooldown lowers every time it jumps to infect a new enemy. It’s especially potent when paired with , because biting an enemy who’s already been infected with will stack an additional instance of the Poison.
Keep in mind, this is a flex slot that should be replaced ASAP if you’re struggling to survive in whatever content you’re running - , , and are all excellent Defensive options.

An absolute staple in many builds for the massive boost to Damage Reduction, and it’s fairly easy to keep the bonus up at all times. The Poison Summoner is going to be swiping away with whenever there’s nothing else to do, so this will have a near permanent uptime. This can be moved to your Helm Slot if you want to use here.

This aspect is fantastic for boosting all damage. It’s a perfect choice for the Poison Summoner, because the build doesn’t rely on any Spirit spenders. Your Spirit will always be at maximum, thereby ensuring a nearly permanent uptime on the damage granted by this aspect.
This is a flexible aspect that can be swapped out for your preferred Offensive or Utility Aspect without hindering the build.

Not all Companion Druids have warmed up to this Unique yet, but now’s the time to take a second look. This item will transform their Physical damage attacks into Lightning damage, and will grant the skill activation a small AoE Lightning explosion - as a result, your good boys will now trigger the Spirit Boon, Overload! Additionally, the innate Item Affixes are incredibly strong for the Poison Summoner (the bonus skill ranks are worth their weight in gold). There’s no way around it, this Unique is an absolute must for any build investing heavily into .

Here’s an aspect that we can truly appreciate - it grants a

Movement Speed boost to the wearer when moving away from enemies that are Slowed, and thanks to , enemies of the Poison Summoner are nearly

Slowed. There are other options for this slot, but we prefer this aspect when Movement Speed is the priority. That said, feel free to forgo a Mobility Aspect altogether in favor of another Utility Aspect here if you’d prefer - is a strong defensive choice.

This aspect is right at home on the best in slot wishlist for the Poison Summoner. This grants one additional
Wolf
and one additional
Creeper
boosting their overall damage potential, and also grants them a significant multiplicative damage boost - don’t leave home without this one. Unless you haven’t found it yet… in that case -
get out there and find it!
This is a must-have.

Grants the Attack Speed and Damage buffs from to your Companions. This significantly boosts their damage output, and will do so without any additional effort, as you’re already going to be keeping these buffs up anyway to boost your own stats. Interestingly, because all Companions receive 100% of their master’s stats, this aspect allows your army to effectively double-dip into the Passive Skill - Companion stats are boosted when their master gains the benefits of and then again when they receive the buffs themselves. This is a very strong aspect for Companion builds, and shouldn’t be overlooked.

This powerful Unique has been through several iterations, and it’s definitely not as strong as it once was. That said, it’s still a solid option for Damage Over Time builds and fits in the build like a glove (or ring…). Besides boasting some impressive innate Item Affixes, it grants every single tick of every single instance of Poison a chance to deal a large burst of damage on Lucky Hit. The Poison Summoner is in a strong position to take advantage of this powerful effect, because the build already relies heavily on generating Lucky Hits to reset Companion Skills via the Spirit Boon, Packleader. We recommend this Unique to help smooth out the flow of combat when you hit a “skill reset” drought and your Companion Skills are sitting on cooldown.
Keep in mind, this is a flex slot, and you can swap this out without substantially weakening the build - , , and are some solid alternatives.
BiS Main Hand for Poison Summoner Druid
Aspect of the Alpha

This is a must-have for the Poison Summoner, so say “goodbye” to the handsome little good boys that accompanied you prior to this aspect and “hello” to the fierce Werewolf Companions that join you after. The beef-ification of the Companions isn’t purely physical - this powerful aspect will skyrocket the damage of your and allow them to cast often. We strongly recommend imprinting this aspect onto a Two-Handed Weapon to double its effectiveness, but you can get away with putting this in your Amulet Slot instead if you’d prefer to use another weapon. You can’t escape this aspect though - not including this one would break the build. Use it, and love it.
Best Item Affixes, Tempering, and Mastercrafting
In many cases, you will want to re-roll at least one undesirable Item Affix from a piece of gear. Unfortunately, this is not possible with Unique items; all Affixes are pre-set, and you aren't even allowed to drag it into the Occultist's Enchant window.
You can only get a Greater Affix as a drop; replacing it at the Occultist will replace it only with a normal Affix, not a Greater Affix.
These are among the most desirable Item Affixes, Tempers, and Masterworking for Poison Summoner Druid in endgame, organized by type (not all Affixes can appear on every type of gear). Remember that you can only reroll one normal Affix per item, so if you get an item with more than one undesirable or unusable Affix, you may want to reconsider using it.

Alternate Gearing for Poison Summoner Druid in Diablo 4
Since you are not always going to be able to get the exact pieces you need, or you get a lucky drop of something else that looks interesting, we present some alternative Unique, Farmed Legendary, and Dropped Legendary choices. These are especially important when considering any Unique or Dropped Legendary BiS gear items you may not have been able to acquire.
Alternate Unique Items for Poison Summoner Druid
Since Unique items are all drops, you can't guarantee getting that one item that you're looking for. Here are some possible alternatives you can use.
- If you find this and want to use it, you certainly can - because you’ll be spamming when your other skills are on cooldown, the Poison Summoner can make effective use of this weapon. It does have its benefits, namely increased survivability, but you’re sacrificing a lot of damage for that increase. If you want to use this weapon, move
Alpha
to your Amulet Slot, ditch
Edgemaster’s
, and imprint
Stampede
onto your Gloves.
- Using this Unique could be fun, and probably quite effective! If you want to try this one out, swap
Alpha
to your Amulet Slot again and put
Stampede
in your Totem Slot. You should also drop or to pick up instead.
- You may want to use this item if you find it due to its strong innate Item Affixes. It likely won’t outperform a Legendary in this slot, but it’s certainly not a bad option. If you use this, move
Might
to your Helm Slot, and either drop

or move it to your Gloves, dropping

- These Gloves have amazing innate Item Affixes and a very strong Crowd Control effect. If you’re struggling to survive, see what happens if you start slapping enemies with these babies equipped - you’ll Freeze baddies faster than you can say “chill out.”
- This is an amazing item for pulling in and grouping enemies, but the Poison Summoner doesn’t really need it due to the massive area of effect of . However, this is an
awesome
Unique for supporting a group. If you’re playing with friends who can’t pull in enemies themselves, equip this item, often, and they’ll love you for it - in that case, you’ll also want to swap your Boots out for .

Alternate Farmed Legendary Aspects for Poison Summoner Druid
Particularly early in your character's endgame career, the "best" may be elusive and some slots will need to be filled with items created by farming Legendary Aspects. Here are some of the best Legendary Aspects for Poison Summoner Druid and where to farm them.

Alternate Dropped Legendary Aspects for Poison Summoner Druid
Dropped legendaries are a little trickier, since ideally, you'd find one that's already high quality and on the slot you needed so you don't need to Salvage it to your Codex right away.
- This is an awesome aspect if you find it and want to use it. It grants a stacking Attack Speed buff that supercharges at 5 stacks to grant a Basic Skill damage bonus and a Movement Speed boost. We’re not super reliant on the damage of our Basic Skill, so it’s not best in slot. That said, the Poison Summoner can use this aspect very effectively.
- Another relic from Season 2, this aspect is actually fairly valuable if you want to add a source of Vulnerable to the build. Between and your Companions, you’re able to trigger its effects quite often.
- A decent aspect for boosting specific damage types on a rotation. Because Lightning and Poison are on separate rotations, you’d want to activate during the Lightning rotation and during the Poison rotation.
- This aspect can be helpful for boosting the duration of your Crowd Control effects. It also acts as a pseudo-execute mechanic by significantly increasing Critical Strike Chance against Injured enemies. Your mileage may vary with this one, but it’s not a bad aspect.
- This is an excellent alternative to if you’re in need of additional Armor, as long as you can live with a longer cooldown on your Evade!
- Feeling like you don’t have enough self-healing? Look no further than this aspect.
& - If you feel like enemies are frequently one-shotting you, combine this pair of aspects with a healthy dose of Dodge on your gear, and you’ll start to survive one-shots by simply avoiding them.
- This aspect can help lower the cooldowns of your Companion Skills if the Packleader Spirit Boon just isn’t cutting it for you.
- Provides a Druid with the ability to Daze targets, which can be a valuable form of Crowd Control. The Poison Summoner can use this well thanks to a high Lucky Hit Chance.

Best Gems for Poison Summoner Druid in Diablo 4 Season 4
Gems come in various qualities from Chipped to Royal. While the gem properties do not change, the traits improve with higher quality. Therefore, while we list only the highest quality gem here for listing purposes, you should always socket the highest level gem you can get and equip at your level.
Best Armor Gems for Poison Summoner Druid
Now that
Sapphires
grant Willpower, they’re the best option for slotting into Armor from a damage perspective. However, if you’re struggling to survive, use reliable old
Rubies
here instead to increase your Maximum Life. More life = longer aliveness - seems like a win.
Best Weapon Gems for Poison Summoner Druid
As above,
Emeralds
are going to be your best bet here - inherit their master’s Critical Strike Damage, so these are a direct boost to your primary damage source. As far as alternatives go, try using
Skulls
here if you feel like you don’t have enough self-healing. Nobody ever talks about using these Gems in a Weapon, but the healing rate isn’t half bad and improves as your kill speed improves - of course, you’ll sacrifice some damage.
Best Jewelry Gems for Poison Summoner Druid
What Gems you decide to socket into your Jewelry will largely depend on your current Elemental Resistances. We’re recommending
Diamonds
here as a placeholder thanks to the Resistance to All Elements they provide, but use what you need here - if you’re low on Fire Resistance use a
Ruby
, Lightning Resist use a
Topaz
, etc. The goal is to reach maximum Elemental Resistances (70%) in your current World Tier. If you’re already Resistance capped,
Skulls
are a great alternative to boost Armor.
Gems Guide for Diablo 4
Best Elixirs for the Poison Summoner Druid in Diablo 4 Season 4
Elixirs offer a wide variety of effects, along with a 5% boost to Experience. These can be either offensive or defensive, and a few of them are so powerful that you must find the recipes out in the world to learn them.
Many Elixirs are situational. If you're fighting the world boss Ashava, a can be your best friend, or an can be just the thing for a dungeon you're looking to speedrun.
However, there are also good "running around" Elixirs you'll want to keep on hand for general activities. For the Poison Summoner Druid, we recommend the following Elixirs for everyday use:
- If you haven’t capped out your Attack Speed yet through other means, this Elixir is going to be your preferred offensive choice until you do.
- If you’re capped out on Attack Speed, the Critical Strike Chance and Damage boost gained by drinking this Elixir is absolutely going to be best in slot.
- This is an attractive defensive Elixir if you’re not yet Armor capped, with an added bonus of increasing your Thorns slightly (and therefore, your Companion’s Thorns).
- Drinking this Elixir grants a
huge
boost to Maximum Life, making it fantastic when you’re looking to beef up your already beefy Poison Summoner.
- This is a great Elixir for consistently increasing your Movement Speed, and should be considered anytime you’re speed running less challenging content.
Consumables Guide for Diablo 4
Best Incense for Poison Summoner Druid in Diablo 4 Season 4
Incense works both grouped and ungrouped, and increases stats, resistances, or defenses for 20 minutes for all nearby players.
- This is your best in slot Incense, both solo and in a group, as it provides a bonus to All Stats and a boost to Critical Strike Damage. It is rather costly in terms of crafting materials, so please burn responsibly.
- For everyday personal use, this is a good option to boost Willpower, and it’s slightly more cost effective than the above recommendation.
- Stack this on top of and and you’re in business.
- Increasing Maximum Life is always valuable for survival, but keep in mind, because this Incense gives a flat amount of Life it’s not nearly as strong as its Elixir counterpart.
- As with Elixirs, Incense can be used to shore up weaknesses in Elemental Resistance - that’s the best use case for this one.
