This one, and the part 4 isn't spawning for me either. I finished the campaign and all the side quests, and every mob is neutral to me now, not sure if that has a part in it, but very annoying.

I couldn’t see the item, I even logged out and came back two hours later, still nothing. So I went ahead and completed the follow-up quest “The General’s Resolve” and turned it in to get the achievement “The General’s Salute”, and then the item appeared, (the fourth one too). I had checked before turning in the quest (so without the achievement), and still couldn’t see it, so I’m not sure if it’s just a coincidence or not, but it definitely seemed connected. Good luck!

So it would appear you have to complete the achievement "The General's Salute" before 3 and 4 appear. I got all of them, then found the general selling car insur... wait, wrong general. Found General Vizier with a quest, did his quest, and bam they appeared. Hope this is helpful.
