Right click NPC - Left click shop menu - Right click thing to buy - confirm - close Shop menu - open bags - right click chest containing item - right click item - repeat.

To save some time, you can speed up the process a little bit with a macro. This will open the cache you bought while the vendor window still opened:

I sat and bought all the chestpieces.
No duplicates.
Once you have them all, a message prevents you from buying again because you know all the looks.
The only annoying parts is you have to buy and unbox them one at a time and the RNG, but at least no duplicates.

One of the dumbest vendors ever added in video games, one par with how some seasons were in D4.
You can only buy 1 chest per slot at a time.
You can only open the chest if you have learned the item that is inside.
You have to close the vendor window to be able to open the box.
Who does these, jesus ...
