DPS boss. Physical fight. Patchwerk has one devestating attack that will one-shot most tanks if they do not have most, if not all of their Tier 2 gear. It's called Hateful Strike. It was designed to hit most clothies and leather wearing classes for ~28,000, but will be mitigated with armor. Tanks usually can absorb the blow with ~10,000 armor which brings it down to ~4000 to 9000 damage. Healing rotation is key here. Three or four healers will have to rotate big heals when Patchwerk does his Hateful Strike. The attack occurs every 1.5 to 2.0 seconds.
Everybody else in the raid is burning their cooldowns and just attacking as fast as possible to get Patchwerk down. After 7 minutes of fighting, he goes into Enrage mode and starts hitting for ~50,000 to ~120,000 w/ crits ~200,000+. As you can see, nobody will last long at that stage. The approximate raid DPS required is ~10,000 DPS. Anything less will result in a wipe.
Patchwerk will go into a normal Enrage mode I believe at 20% health. This is similiar to Battleguard Satura's enrage and with most bosses before as well. Easily manageable.

A Mage's Guide to Patchwerk (Mana Management)
The key to this fight is mana management. This probably will not be as much of an issue while the healers/tanks are learning their roles, but it will be an issue once they have it down. During a near perfect fight (7-7:30) the following will be used:
Mana Ruby, Demonic/Dark Rune, Demonic/Dark Rune (If we are good on dps Mana Citrine and Mana Agate can be used here instead of 2 Demonic/Dark Runes)
Major Mana Potion x3 (If we are good on DPS, Superior Mana Pots can be used here)
Evocation x1 - To maximize the amount of mana received here we should switch to a spirit staff with +22 spirit enchant on it. This will add more than 500 extra mana to evocation. Here is a link of staves with more than 20 spirit

Dancing Sliver would be easiest to get (scholo quest) or Soulkeeper from the AH can suffice. Ideally, Will of Arlokk would be the best choice here. Switching weapons has a 1.5 second global cooldown but it CAN be done while casting a spell. So on the last Fireball/Frostbolt before evocating, switch to the spirit staff and on the first Fireball after evocating, switch back to your normal 2hander or 1hander + offhand.
Also, Celestial Orb (lvl 40 mage quest) can be used. The same strategy with switching it in/out while casting Fireball/Frostbolt will apply.
Now, to spell usage.
Fire Mages will use a mixture of scorch and fireball. There are 2 levels of mana management here. The first is actually having enough mana to chaincast for 7-7:30 minutes. The second is chaincasting as many fireballs as possible. Scorch's natural superiority to Fireball in mana efficiency is amplified by Judgement of Wisdom(2 scorchs in the time of 1 fireball - double the mana from JoW) and clearcasting(can fireball on scorch's clearcasting proc). The ideal(but not achievable) damage here would come from chaincasting only fireball for 7-7:30 minutes except for reapplying imp scorch debuff, but that would require perfect mana management, all the best consumables, and the best gear possible. Even then it seems unlikely.
Frost mages basically spam frostbolt for 7-7:30 minutes.

I also found it useful to use Mageblood Potions or any food with +Mana/Sec buffs and or spirit. These are both very effective since this is a very intense mana fight. If your spirit is low and so is your intellect, try spamming pyroblast for a while its the most effecient DPM spell mages have. Also use Mage Armor the regen is a MUST!

Hunter specific tips:
Ofc get all the kinds of dps and mana increseing items you can get a hold of except Flask of Distilled Whisdom. Hunters can keep their mana up by just using Major mana pots and runes. If you got a shaman in your group you only need to use Major manapots.

He enrages at 5% not 20%. At his time-based enrage in addition to increased physical damage he will decimate your raid in a matter of seconds with AoE poison bolt spam which was put in to prevent kiting.

Priest Tips for Fight:
Make sure you guys have tons of Mana pots, Nightfin soup helps a lot and use your low rank spells like Greater Heal 1 and Heal 4.

As a rogue, this is a great way to test your DPS and find where you stand. Properly specced, rogues should be within the top 5 dps for sure. It is a great benchmark to improve ones stationary DPS. With the new 2.01 patch, combat potency is a must have talent that will definetely up your dps as well here.

This fight is alot easier then people expect it to be, its a simple tank and spank fight. We were expecting it to be very difficult at the time had 5 bosses down in naxx, and we took it down the fire real attempt, everyone had flasks. We were surprised with how easy it really was. Recommend having the tank with lowest HP be MT and all the rest of the tanks have higher HP so they take the hateful strikes. Paladins on MT and everyone else use highest heals on the other tanks.

After 2.0 Patch he was hitting for 13-14k on 8/9 Dreadnaught tanks. This was hotfixed, however with the rating system implemented he still hits somewhat harder. The fight is certainly doable still but it is a little more rough on the healers.

1. Hateful Strike Mechanics
In order to understand the fight you only have to understand the only variable of the fight and that variable is the Hateful Strike.
- When Patchwerk is engaged by the MT he becomes highest on the Aggro list. This mechanic is similar to the Twin Emperors ->Proximity Aggro is gives a huge aggro bonus. The MT does NOT recieve the "Hateful Strike" Primer.
- The three closest players to Patwerk recieve a "Hateful Strike Primer".
- The player with the highest hitpoints and "Hateful Strike Primer" will become the target of a Hateful Strike.
2. Healing Strategy
The Healing strategy is the ONLY real strategy for this fight!!!
* MAIN TANK
The MT does not recieve "burst" damage. He recieves a steady 1000-1200 DPS. Healing for the MT should be a combination of Priest Heal Rank 4, Druid renews and regrowth, and Paladin Flash Heals. So, 4 Healers: Priest/Druid/2Pallies or Priest/2Druid/Pally
* OFF TANKS
The OFF TANKS will take a large "burst" damage every 3.75 seconds. Expect 8000-9000 damage from the Hateful Strike every 3.75 seconds.
OFF TANK 1
- OFF TANK 1 should have the highest hitpoints, highest mitigation, and highest avoidance of all 3 tanks.
- If the healing is right on this tank he will recieve the most Hateful Strikes.
- A priest using GHEAL should be landing a heal on this tank every 2.5-3 seconds.
- A Druid should be landing a regrowth on this tank every 2.5-3 seconds
- A Paladin will use Flash Heals (1.5 sec cast) to keep this tank topped off.
OFF TANK 2
- OFF TANK 2 will be the next geared OFF TANK
- If the healing is right on this tank he will recieve the second most Hateful Strikes.
- A priest using GHEAL should be landing a heal on this tank every 3rd second. (Cancel if topped off)
- A Druid should be landing a regrowth on this tank every 3rd second (Cancel if topped off)
- A Paladin will use Flash Heals (1.5 sec cast) to keep this tank topped off.
OFF TANK 3
- OFF TANK 3 will be the next geared OFF TANK
- If the healing is right on this tank he will recieve the third most Hateful Strikes.
- A priest using Flash Heals every 1.5 seconds.
- A Druid should be landing a regrowth on this tank every 3rd second
- A Paladin will use Flash Heals (1.5 sec cast) to keep this tank topped off.
Summary
The healing strategy should be set up to keep the Highest Hitpoint tank topped off every time he takes a hateful strike. The result will be that we can predict which tank takes the hateful strike and keep him topped off.
If a OFF TANK dies the most likely heal team to blame will the Heal Team of the OFF TAnk who is "supposed" to take the next "Hateful Strike"
If the order is:
A, B, A, B, C, A, B, A, DEAD A - Heal team B and C were sucking since Tank A had the most hitpoints and took 2 in a row.

- THe melee and ranged attackers will be due south of the MT group.
3. Fight Dynamics
* Initial contact
The MT will aggro Patchwerk with a ranged pull. The Raid will wait until Patchwerk uses a Hateful Strike to engage.
* Fight Strategy
- ALL OUT DPS ZERG - OPEN UP ALL THE STOPS AND LAY DOWN MAX DPS.
- Healers keep the MT and OFF TANKS alive and the fight is over.
* Enrage
- At 7 minutes into the fight Patchwerk permanently Enrages.
* Consumables
- Stoneshield Potions - The MT and OFF Tanks should have a huge supply of these and drink them like water (whenever cooldown up)
- DPS Enhancing Buffs - Anything to increase your DPS needs to be brought to this fight.
- Supreme Power Flasks - All Mages/Locks should be flasked up. Send the bank Dreamfoil, Silversage and Black Lotus and we will provide the Flasks.
- Titans Flasks - The Guild Bank will support Flask of the Titans on the MT's.
* Important Debuffs
The following DEBUFFS should be maintained on Patchwerk at all times:
- Insect Swarm
- Curse of Weakness
- Thunderclap
- Demoshout
4. Special Notes
- The key to this fight will be keeping the healing rotation on the Off tanks.
Extra - Overhealing is a killer heal, make sure your timing your heals perfect on a rotation and that your dps is sick.

As an addition to all the above information, Patchwerk was modified 1.12.1 to have another Enrage trigger at 7:30 in to the fight. At that point in time, he will cast a 360 degree AoE Slime Bolt for approximately 25,000 damage. This change was most likely implemented to deter any attempts at kiting Patchwerk.
To reiterate what has already been pointed out, Hateful Strike is the mechanic of focus in this encounter, and though understanding and adapting to the primer and the hitpoint mechanics can be done easily enough, the crux of the difficulty of Patchwerk lies within healing through it.
The most popular HS strategy is done with 3 offtanks, but there are of course deviations from this, (mostly on alliance side engagements, with them using less oftanks - this is usually due to healers' longevity in regards to mana) and every guild should find what works best for them and their healers. I will touch on the 3 offtank strategy as it is the one I am most familiar with.
To start, here's the lowdown on Hateful Strike. It is a flat damage range. That means that it cannot crit, it cannot crush and that it is not modified by ANY statistics whatsoever (which means CoRecklessness is okay if you can handle the increase in damage on the MT). As well, it hits approximately every 1.2 seconds using the mechanic described above. From there, let's look at the numbers:
Minimum Damage: 22100
Maximum Damage: 29900
Assuming a fairly well geared warrior, let's give him some attributes. To help us kill Patchwerk, remember that we'll be using Greater Stoneshield Potions, which give 2000 armor to our tanks. We'll forget about any superfluous armor additions from things like Grace of Air or Elixir of Greater Agility (though these are worth using!).
Armor: 9500 + 2000
Block Value: 250
Now, there's also the possibility of Inspiration or Ancestral Fortitude being up on your tanks, which modifies total armor value after all other things are taken in to account. We'll spit out some calculations for the armor increase being up and not being up.
Armor w/ Inspiration/AF: 14375
Armor w/o Inspiration/AF: 11500
Let's go ahead and calculate how much damage your offtanks will take using this formula:
max = ((29900 * (1 - armor / (armor + 5755))) * .9) - blockval
min = ((22100 * (1 - armor / (armor + 5755))) * .9) - blockval
Notice that we're also assuming that our warriors are in defensive stance.
With Inspiration/AF:
((29900 * (1 - 14375 / (14375 + 5755))) * .9) - 250 = 7443 MAX
((22100 * (1 - 14375 / (14375 + 5755))) * .9) - 250 = 5436 MIN
Without:
((29900 * (1 - 14375 / (11500 + 5755))) * .9) - 250 = 8725 MAX
((22100 * (1 - 14375 / (11500 + 5755))) * .9) - 250 = 6383 MIN

Now with that out of the way, we might be able to figure out exactly how to heal through that. As has been pointed out, you want the best geared warrior to be the first target in the HS rotation. Probably the most important stat for him or her (aside from enough mitigation and health) is avoidance. Avoiding damage is a huge boon to healers, as it effectively gives them 1.2 seconds of free healing.
However, most of the time, your warriors won't be avoiding HS, and you must heal through it. From a horde point of view (where mana issues tend to be greater than alliance side), when using three offtanks, it's usually most effective to have three healers per offtank. Depending on how many healers you roll with (I'd recommend 16 for those just starting the encounter), you can allocate more to the HS tanks as needed, but make sure to not forget about the MT and his damage intake. It is a common error of many guilds learning Patchwerk to focus so much on their HS offtanks that they often wipe due to the MT dying.
If you have 16 healers, I would recommend using the following setup for those just starting out with the encounter.
MT: 5 healers (3 shaman, 1 druid, 1 priest)
OT1: 4 healers (2 priests, 1 shaman, 1 druid)
OT2: 3 healers (2 priests, 1 druids)
OT3: 3 healers (1 priest, 2 druids)
As you'll note, the above only includes 15 healers. It also allocates a total of 6 priests, 4 shaman and 5 druids. For your 16th healer, I would actually recommend a shaman. Not only do you get another shaman for mana regeneration through totems (or DPS totems if you have that role filled), but you also get another healer that can proc the armor increase through Ancestral Fortitude. Personally, I would use this healer on the second offtank or on the MT himself, but again, learning the encounter highly depends on getting familiar with HS, so allocate him as needed.
There are several reasons to use this healing setup, but the most notable are as follows:
1. Enough healing on each offtank to sufficiently heal HS damage within 3 seconds of taking it.
2. Controlled HS mechanic offloaded to three warriors.
3. High percentage of having Inspiration/AF up at all times on 3/4 of the total warriors needed
4. Promotes and improves the skill of good healers
As for what healing spells each person should be casting, that really depends on your warriors. Use the formula to calculate how much maximum damage your HS warriors will take and work from there, making sure that your healing groups can always cover the top-end of the damage.
Once you get familiar with healing through HS, your point of view on dealing with it may change. This healing strategy is by no means the only one (there are especially different make-ups on the alliance side as I mentioned), and most assuredly not the best, though I think it is a solid way of starting out with learning the mechanic.

I officially declare Patchwerk a true Tank-and-Spank fight. For all you out there, like me, who would always want a boos who was just hardcore with not many special abilities, just brute strength and a push for time... this is or at least was the fight.
Anyway, now that he will be level 80 content I expect he should be a piece of cake. Kind of sad to see all of these encounters moving to 25/10-mans

Killed him last night in a pug. I'd suggest the tanks having at least 30k hp buffed. Try to reduce his incoming damage as much as you can, even if it sacrifices a bit of DPS (like using Curse of Weakness and whatnot).
In the 10 man version, the enrage timer is pretty generous, not sure about the 25 man, but I'll assume it's the same.
Also, if you have any mages, make them put Amplify Magic on the tanks.

Raid setup:
You need 2 tanks. Dosent crit, but hits for rogue 50k+ so you need some avoidance (we used prot warrior and "tank" DK)
+6 healers, you go oom rly fast
(we used 2 holy paladins, BoL. One druid, hots. Priests, renew and shamans, lust)
(paladins: use HoS when "enrage")
Hateful Strike hits about 28k(heroics/10man naxx geared tank) pref tanks with +32k hp raid puffed. Healers needs to know that before you go.
Melee: must have less hp than tanks or Patchwerk one shots you.
Healers: you go oom rly fast.
Casters: You cant get agro... go all out.
If you didnt do naxx @ lvl 60 this is not the best "guide". I dont write/speak good english so i dont explain better, but if anyone in the raid have done this before she/he can explain rest of this boss and you can get this one down.

There are two different versions of this fight.
Non-Heroic
Only two tanks are needed. One main tank and a Hateful Strike soak tank.
Heroic
Three tanks are needed. One main tank and two Hateful Strike soakers.
Tanks who are only soaking Hateful Strikes
do not
need to be crit immune. They should focus only on High Stamina and Armor and after that avoidance can help, but is not mandatory.

- Paladin's using Beacon of Light put up great healing.

On 10 man we've been doing this with a warrior MT, and paladin OT soaking the hateful strikes. We've been doing it with 2 paladins and a shaman healing. Both the paladins were keeping beacon of light on the MT which made healing both of them at the same time really easy. All the healers were getting very low on mana when we killed him and had to use all their cooldowns. Melee was doing around 13k raid dps and we got him down with some time to spare. He enrages at 5% doing a rediculous amount of damage and you have to have the tank pop shield wall and last stand as well as shield block. It's also really helpful to have an
Indestructible
Potion up so that you get the buff during the enrage at 5% so do your best to time it accordingly.
-Absolutely NO ONE should be tanking damage except if the heals are slow on the OT getting Hateful Strikes and one of the raid accidentally soak one.
-Positioning isn't particularly important, I'd suggest putting him across the channel of green goo to avoid accidentally getting hit with it and having your stats halved when you engage him. Make sure you have the dps behind him as usual and pumping out everything they've got.

Just want to add we tried him last night in our first 25man, I was the MT as a prot paladin. 32k hp, 29k armor, capped defense. The hateful strike tanks were two druids with ~35k hp, capped defense and ~32k armor. We just couldnt pull it off. his melee attacks in heroic mode are incredibly powerful, regularly hitting me for close to 10k each. We had done every other wing without too much trouble but this guy is just too strong for the moment. We will be trying him again before the reset after we do four horsemen but Im not too sure if we have the gear to take him down just yet. Just be warned that he hits VERY hard in heroic.

For 25-man, I would recommend druids and DKs to soak the hatefuls because of both classes high stam and armor. Main tank can be any class -- they take the least damage and just need to stay on top of threat.
10-man though we did this no wipes with two average-geared warrior tanks. It's really no different than the old 40-man, just make sure melee dps slime themselves often and don't get heals.

Patchwerk has ~4322000 hp in normal.
You have to kill him in 300 seconds.
You have to sustain 14407 raid dps to do it in time.
So if you have three healers and two tanks, tanks who do ~1k dps, each damage dealer will have to do 2,6k dps.

with 3 healers, each person needs to put out roughly 1716 dps. Considering the 2 tanks will have lower single target dps, you are looking at dps having to push 1.8k-1.9k which allows your tanks to have 1.5k dps.
Realistically, you want your dps to be pushing 2k dps.

Paladin healers should always be assigned to the Hateful Strike tanks.
We used:
Tanks:
1 Warrior MT
1 Warrior OT + 1 Paladin OT.
Healers:
3 Paladins
1 Shaman
2 Priests
1 Druid
The Paladins basically keeping Beacon of Light on the OT's and normal heals on them. Rest of the healers on the MT.
Our raid was geared like 50% blue questrewardgear and rest a mix of badge / naxx 10 man epics. And our raid DPS was 50k, which is 14k above the required raiddps. So any guild with the same gear should easily be able to pull this fight off.

: Go all out. He will enrage at 5% and go beserk after 7mins. This is a dps race.
Healers
: Keep big heals on OT at all times. MT will take steady even damage and the raid will take minimal to no dmg. We put the MT healer on the hateful-soaker and the raid healer on the main tank.

Everyone keeps saying that it is a 7 minute enrage timer. It is not.
Its a 360 second enrage timer, not 300.
Berserk - After 6 minutes of combat Patchwerk will go Berserk, increasing his damage by 500% and his attack speed by 50%.
The enrage timer is 6 minutes, not 7. Most of the Naxxramas stats/strats listed on WoWhead have not been updated to their new 10/25 man stats/strats. If any of the moderators/editors/whatever read this, it might be a good idea to update everything. (WoWWiki has updated strats/stats on their new Naxxramas page at

A few notes:
A pally tank generates insane threat per second on this boss, due to the sheer number of hits on them.
Most tanks will generate so much threat that you cannot catch up to them as a dps...meaning you can go all out almost immediately.
As a healer you have to keep very very sharp; in the 10 man, our pally tank went ahead of me (feral) and in about 2.5 seconds was almost dead! Just remember that his guy plays no games.
Someone had recount where it showed the damage patchwerk did...even with all of my dodging and parries with my deathknight friend in the 25 man he pulled over 7500 dps. Also, the hatefuls were hitting me (as a decently geared bear) for about 14k a pop.
Overall, if you have a gear an amazingly fun fight as dps since you can truly go 100% and not really worry about threat. Also, tanks will help dps a lot since you have pretty much unlimited rage during this fight (and sanctuary I assume is nice for pallies since you get so much of your mana back when you block/dodge all of those numerous attacks). Have fun with him!

Beat this boss last night on 10 man. First 2 attempts we hit the enrage timer at 500k health or so. We had 2 sub par dps with us unfortunately. However even with them we did manage to beat him.
First of all patch is a pure dps race. Your dps probably has some long cd abilities they're not using. I know that i had to remind a warrior to use his cd abilities. If your dps isnt great remember to bring 2 shamans. We had 2 and managed to do pop a second heroism straight after debuff faded to get him down.
Tank wise regardless of who has most health you must remember your tanks are doing dps and unlike the other dps classes threat will hold them back. Luckily for me as a pally tank i had less health than our mt warrior =). Paladins do unbelievable threat here as a mt and will struggle as an ot. I was putting out 1800 dps on this fight due to undead only abilities and unlimited mana. This is a huge upgrade from my general 1.2k or so boss dps. This dps translated to over 5k tps so no dps was getting anywhere near me and my ot warrior was doing about 3k tps.
Basically this fight is making your dps use their cds and trinkets. Patch is unlikely to cause you to go oom on 10 man as a healer and the damage is relatively predictable. This is a pure dps race and your dps have to treat it as such.

.
This ability does not hit a target in melee range with the highest HP - so
DO NOT DIP INTO THE SLIME, DO NOT CLICK OFF YOUR BUFFS!
Instead, Patchwerk does this:

So, you want your 3 tanks to be on top of threat - which becomes really easy after a few hatefuls because of the free threat boost. A good strategy is to allow your tanks about 3-5 seconds to prime threat (and allowing 3-5 hatefuls to hit) before allowing melee DPS to begin. This will ensure that your tanks have a huge threat lead, and that none of your melee will get instagibbed.

I would highly recommend the Main Tank on 25man pop cooldowns such as Shield Wall, Icebound Fortitude, Survival Instinct or Divine Protection on the pull. Sometimes the offtanks do not react fast enough to prime aggro on Patchwerk and, with the melee not yet in, the MT eats a Hateful. If he's not ready for it, he'll probably be killed in less than 2 seconds, whus wiping the raid on the pull. The offtanks should save their cooldowns for the last 5%.
Blowing your cooldown this early can have 2 possible consequences:
-The 5% enrage hits before the cooldown refreshes: This means that your raid bursted Patchwerk before 5 minutes, meaning you have good dps. Without these abilities, there is a chance that the MT will die. Not to worry, since the OTs should have popped their cooldowns (saved for this part of the fight) and can take on patchwerk until the dps brings him down (which should be possible considering they brought him to 5% in less than 5 mins.
-The 5% enrage hits after the cool refreshes: You'll be able to reuse them for this part of the fight. All normal.
**DKs have it easier, since Icebound Fortitude has a lower cooldown**
Tried this yesterday, after the MT dying twice on the pull due to server lag. It worked perfectly.

First, I wanted to confirm. The hatefuls are targetting the players with the top 3 HP in melee range regardless of threat? Or is it the player with the highest HP that's also top 3 on the threat table? (We've been having our melee bounce back and forth from the slime to avoid getting hateful. Is this necessary?)
Also, I'm surprised this hasn't been mentioned yet. (Or maybe I missed it?)
The spell Ampify Magic is an excellent way to help the healers out on the tanks, as there is no spell damage taken in this fight.
Conversely, if my understanding of the mechanics is correct (from my question above), same would go for applying Dampen magic to the melee DPS group to mitigate the unavoidable heals they will undoubtedly take from passive AOE healing talents.

Has anyone tested whether or not Hateful strikes are dodge/miss/parryable? I've been stacking pure stamina on my druid, which seems to work reasonably well, but I'm going to start moving toward more avoidance. I hope its not going to screw up this fight.

I can't recall reading this anywhere but does anyone know if hateful's can hit hunter pets? When we first tried him the other night, I was OT (cause I have the highest HP) and lagged a bit before the pull so the MT ate a few hateful's before I got in range and died pretty fast. I then Tanked him all the way till his enrage without even coming close to dying and I was the only other melee other than hunter pets, but still had the highest HP compared to the pets. Can the hateful hit the person with the highest threat if they have the highest HP if there are pets in melee range? I didn't check my combat log but I think I got hit with the hateful's. Anyone else had this happen?

From what I've seen of this, we can glean that he has no magical abilities, that this is a race to beat the Enrage Timer, and that this guy hits like a fully-loaded Pepsi truck. However, just trying to make this clear, from what your guide says, his Hateful Strike occurs at a 1.2 second frequency. Is this true?

Today is patch day for 3.0.8 and as we got to him, we were noticing that he is hitting a lot harder than he was last week. I'm a prot warrior that's pretty well geared, and compared to last week's 18k hits, he is now hitting for 10k more approximately.

With decent gear and very good players you can get away with two tanks and four healers on heroic mode. This does require melee to dip, but wiping buffs is not necessary.

So if I do the math right (10-man):
4,320,000 HP in 3 minutes is 1,440,000 per minute
which means to beat him for the Achievement, your raid as a whole has to be doing 24k dps.
given you probably have 2 healers to start, that means 8 people have to average out at 3k dps in order to down him in time.
If you aren't going for the achievement, you could probably do it with less dps easily, providing you have strong tanks/healers. The rest should go fine. Most tanks that I've read about are doing around 2k dps themselves so... making up for the other 6 spots shouldn't be that big of an issue anyway to do it in under 3min.
Just have to gear up those healers/tanks, and it should be straight forward.
Addition: We actually tried patchwerk for the first time (none had really ever done it before), and we one-shotted him. The highest DPS was 3k and lowest was 600dps. We had around 40 seconds to spare before his big enrage. Not too bad. I think we'll be doing it every week even if it is just as a final attempt at loot after wiping a lot on other wings. It's easier to do patchwerk in the 10-man as some of the other bosses in Naxx. Gothik takes a lot of coordination and dps ... we won't be doing that for a wee bit yet. :D

This boss is one of the most simple of Naxx.
Simply Put ur Tank and Offtank in to get the aggro first when u encounter him, his strikes will now hit them, then its nothing else than a nice tank and spank fight all the way till dead, but its a MUST that the tanks must be able to survive a very Tough damage amount, he hits like 10k per hit, and have a freaky attack speed, in addiction to that he will enrage and increase damage and Speed

Here's a suggestion for both 25man and 10man: If you want to be on the safe side, have your tanks go in with their defensive coldowns activated, specially in the 25man version. Patchwerk will start throwing hateful strkes the second he gets in melee range, so if your OTs are slightly out of place, or don't get on his aggro list, or get ahead of the MT, or something like that, it's possible that someone will get a hateful when they weren't intending, resulting in a quick death.
Go in with your cooldowns activated. Having that couple seconds to establish aggro and hateful rotation will really help

i was in naxx last night on my warrior and the OT died and i ended up taking all the hateful strikes and normal melee hits - he did unreal burst he was pulling at least 10k dmg every 2 secs - so healers make sure to keep both tanks alive or 3 if u on heroic - the OT died at 80% and we got him down to 30% when we hit the enrage - then obviously we wiped

A quick note for those struggling with Hateful Strikes on 25-man...
The natural reaction to having 1 OT die repeatedly here is to add more healers to that OT. This is the wrong thing to do on this boss. Instead, you need to add more healers to the OT that is NOT dying.
Here's why....
He Hateful Strikes every second. In order to keep this going reliably, you need to alternate the strikes between the 2 OTs. If one of the OTs dies because he gets 2 hateful Strikes in a row, it's because the other OT didn't get healed to full fast enough.
The problem goes like this...
00.00:01 - Patchwerk Hateful Strikes OT1 for 25000 damage, OT1 has 12,000 health left.
00:00:02 - Patchwerk Hateful Strikes OT2 for 23000 damage, OT2 has 14,000 health left.
00:00:03 - Patchwerk Hateful Strikes OT2 for 24000 damage, OT2 dies.
Here's how it should go:
00.00:01 - Patchwerk Hateful Strikes OT1 for 25000 damage, OT1 has 12000 health left.
00:00:02 - Patchwerk Hateful Strikes OT2 for 23000 damage, OT2 has 14000 health left.
00:00:02 - OT1 is healed to full
00:00:03 - Patchwerk Hateful Strikes OT1 for 24000 damage, OT1 has 13000 health left.
.. and so on
Basically, if it weren't for avoidance on the OTs, they would need to be healed to full every 2 seconds, alternating. This would drain healer mana very quickly, thus avoidance is very important here.
Also, a Druid casting HoTs on both OTs can help things out a good bit as well. A HoT guarantees some healing between Hateful Strikes, thus it would be very hard for 1 tank to take 2 straight Hateful Strikes.

This Boss has one Rule, if u follow it, u wont wipe at all:
Two tanks keeping his aggro
Enough Heals
If u Follow this rule, he shouldn't be a problem at all!
But do mind ur tank need more than 24k unbuffed, or its a waste to try, his damage will wipe low healthed tanks.
AND!
Dont use more than 6min! If u do he enrages and tanks wont be able to survive the Damage and speed and all will wipe fast.

A common strategy I've seen is to have your weakest tank MT the fight while the other(s) take hateful strikes. The bursty nature of OTing him means that you'll want whoever has the highest health there to give healers more room to mess up. This does mean that the MT might end up throttling the dps down for a bit, but hateful strike's threat increase should prevent this from being a problem after about 15 seconds. He's less of a gear check now and more of a consciousness check. We consistently kill him with a minute and a half or more left before enrage.

Does anyone have any problems fighting him post 3.1? He nails all the melee without regards to Threat, Max Health, or Current Health. The tanks do fine when its just 3 of them but as soon as the melee joins they just eat the hatfuls instead of the tanks who are 1st-3rd have the aggro. But they never get hit once the melee joins in.

We went in Monday Apr 27th with the same group of healers and Tanks from two weeks ago and it was a wipe fest on patches. We cleared him in one shot with this group before and post blizzard patch fest he Hatefull strikes once per sec for more damage. With like 5 attempts the same group of healers that have done patches a handfull of times in one shot just couldnt keep the tanks up. I havent went back over the combat logs is was just a nightmare of an event. But he seems to be bugged.
I dont know what blizzard is doing but they have really messed this fight up. Has anyone cleared patches since the 3.0.9. Today in fact as im posting this I am waiting for the servers to come back on patch 3.1.1 or we might be going to patch 3.1.2 not sure right now with the way blizzard is going nuts.

Surprised nobody has mentioned this before, but
USE AMPLIFY MAGIC.
This is a pure physical damage fight, including the hatefuls. Amplify Magic on the tanks makes healing that much easier.
We also found out the hard way tonight that you have to clear the room before him of the roaming abomination - the one who is walking around with all the little slimes. We left him this time after many Naxx clears because he was on the far side of the room. Cleared Patchwerk's room, pulled... about a minute later, the abomination shows up, followed by 25 or so respawned slimes, who are apparently social with that abomination. I got to do the first ever kite strategy on Patchwerk, walking backward and thunderclapping to keep the slimes off the healers while they got AoE'd down.

Tanks who are only soaking Hateful Strikes do not need to be crit immune. They should focus only on High Stamina and Armor and after that avoidance can help, but is not mandatory.

I wanted to contribute a little story. I have nothing to say about patchwerk honestly, but perhaps my story will be amusing to some people. Oh, TL;DR version: Scrub guild down patchwerk with 6 people.
I am the GM of a pretty small guild. We're all pretty tight with each other, and we don't do much content-wise aside from occasional heroics. Tonight, however, one of our senior members comments about doing the weekly raid quest, and if anyone else has done it yet. Of course, this got a few people talking, while we don't raid as a guild, several members do occasionally raid with other guilds/pugs. We then noticed that we had 8 people online, all level 80. So we said "Alright, lets try to 8 man patchwerk, he can't be too hard." Majority of the members were geared up in triumph tier9, with a few 245s and 264 thrown around, with about 3 people new 80s with heroic gear, so yeah, we outgeared it immensely. That aside, we prepare to go into naxx and clear up to patchwerk. We pull, and the fight gets underway.
My guild was confident that I could be the lone tank, despite my pleas to have out offtank .. well offtank. Patchwerk comes in and we start unloading all we got against him. Immediately, the 2 melee DPS fall, and one a shaman, and 2 druids in the group, one was resurrected 3 times during the fight, only to fall again, because of hateful strike. 6 of us left, we start doubting out skills, but continue to try to take him down. Time dwindles on, and the enrage timer starts to come in... 1 minute left into the timer, and he still has 1million health. Everyone starts panicking, and as it slowly ticks away, I start getting nervous. 60k health left... and he enrages.. However, right before he enrages, I use shield wall, which kept me alive just long enough to burn out those last 60k health. We, a little casual guild, proudly exclaimed amongst ourselves, We ****ing 6manned patchwerk on our first try.

The description on hateful strike is wrong..
In 10man, it always hits the 2nd highest threat player in melee range, doesn't have anything to do with health.
In 25man, it hits the highest health player between the 2nd and 3rd highest in threat, if both players are low hp, one of the two will definitely die.
This is pretty easy to check, usually when melee dies, the offtanks will always be at full hp, because theyre not getting hit. If you don't believe me, even wowwiki agrees that this is how the mechanic works.

An interesting note for those doing the weekly raid quest,
- if you have only ranged DPS, and no melee DPS, then you can dispense with the second tanking slot altogether, because no one will be in range, so there is no need to have someone soaking up this damage and requiring extra heals.
Because of the massive damage he still does on the main tank though, you will still need someone keeping up some secondary healing. If they're a druid, like our off-heals, then they may even be able to just keep HoTs on you, while still contributing to DPS.
Did this today with a host of ranged DPS, one heals, and one off-heals (the afore-mentioned Druid) and only one tank.
Hope this helps those of you having trouble finding two dedicated tanks and two dedicated healers for the weekly. Hunters are easier to find than tanks or heals anyday XD

with the fact that almost everyone running naxx now is overgeared and just doing it to either help out new-to-80 guildies, or for the weekly raid, is it possible to solo tank Patchwerk, maybe having a high health plate wearing DPS just standing around for the hateful strikes (or maybe even a voidwalker pet?). for other hunters who might be considering a solo attempt on one of those off-days with nothing better to do, let me save you some trouble: don't. your pet will be taking all the hatefuls, which come way too fast to heal through. i think my best attempt lasted all of 38 seconds.

- Kings drums, steelskin flask, str food. Didn't have any runescrolls as they're 100g/ea on my server for some reason.
Reforges/stat priority:
- Mastery, avoidance. Used heroic Heart of Rage trinket reforged to mastery and Symbiotic Worm. Tank gear otherwise. Used Akirus the Worm Breaker instead of tier 2 weapon, the additional mastery outweighed the higher weapon damage. Didn't have 2pc or 4pc (tank is my offspec).
Did it in unholy presence with dark succor glyph, the higher blood shield made it doable. Tries in blood presence were ineffective, incoming damage couldn't be outhealed/out-shielded, though having a runescroll and 2pc might have made it a bit easier. Pulled with army ghouls to take the first few melee swings. Took 6 attempts and was one second from berserk on the kill.
edit: as of patch 4.1 you can't use unholy presence / dark succor glyph anymore due to blood shield not working outside blood presence. Might still be doable in blood presence with full 372+ tank gear.

Basically your voidwalker tanks, you heal him while dpsing the boss. But as I couldn't deal enough dps to beat the enrage timer, I used the soulstone trick, i.e dying and resurrecting via soulstone, to reset his timer.

I just solo'd him as Blood DK Ilvl 377.
Started with army of undead up and pulled him into corner.
I used vampiric blood, rune tap, death strike, blood tap everytime they were available. Only other move used was runestrike. I used 1 mythical healing potion though I could have gone without it but better safe then sorry. Used raise ghoul + death pact combo for a heal once. Kept horn of winter up and used icebound fortitude every time it was up. I had 300 strength flask and a 90hit + stam well fed buff

Managed to solo him as a protection paladin in mostly ilvl378 gear from the new 5 mans. I had to use DPS trinkets and reforge to hit/expertise because i wasn't doing enough damage. I seal swapped throughout the fight, starting with truth until I had lost around 100k health then used insight to heal back up with gift of the naaru. I still reached the enrage timer, however he only had 100k left so i killed him before divine shield wore off. It still took a few attempts as my SoTR kept getting dodged and it seriously hurt my damage output.

Soloed as a 381 blood dk, but in dps gear.
This is a great disadvantage, I didn't have tank gear, or I'd have used it.
I had tried some days ago with 372 still dps, started kiting him going backwards from 3 millions or so, used icebound fortitude and the ghoul after at least 1 minute after the fight started, I died after 3 min 23 from start and he had 1,2 millions left.
I knew I could've done better, but wouldn't have been able to kill him, so waited till 381.
This time I didn't use army at the begin, BUT it was a mistake, as if you use it at the begin they take aggro and you can stack some blood shield; if you use it at 2 mil, like I did, they won't take aggro.
Used icebound fortitude at the begin as soon as my hp didn't come back to full 158k, same for ghoul, sacrificed him at 10 sec of his death to heal 40k more.
Used again icebound fortitude and ghoul when they were ready and when he died I had again like 130-140k.
Kited all the way backwards since the fight started, this reduces his dps, as he won't be able (but you also won't) to hit you while you move, and meanwhile the runes regenerate.
This turned out to be necessary with dps gear, quite sure it's not necessary if done with tank gear.
3 mins and 58, so still need better dps gear for the fast kill achievement in solo.

In 25 man, hateful strikes are hitting me (391 ilvl prot warrior) for 25k, every couple seconds, theres just no way to heal through it without bringing several healers dedicated just to the hateful tank

Solo'd him as a DK in 359/378 tanking gear. I had to go full tank gear to do it. Some tips that might help.
Slowly kite him up and down the aisle (be careful to avoid slime). I found it helped immensely since he couldn't hit me so fast and I had more DS opportunities.
Have you ghoul out and ready to go with 30-40 sec left on its timer so you can pop another one sooner.
Keep bone shield up every min
Keep diseases (especially scarlet fever) on him at all times
If you can, in the room before with all the little slimes. agg them before engaging Patchwerk and use them to build up a massive blood shield. If you're lucky you can engage patchwerk starting with an extra 150k or so blood shield.

After trying to solo the achievements a lot of times, as blood dk in full dps gear, a week failed by 2 sec, another week failed by 3, I got tired, used a 300 strength flask and soloed in like 2 mins 58, so got the achievement.
Still kited ofc, I'm not able to solo him in full dps gear without kiting, that is possible in 380 + tank gear, though.
But anyone tried him solo on 25 yet? I heard that he hits much harder, but haven't tried him yet; anyway 36k dps would be required to beat the enrage, so tank gear is not an option.
All the rest of naxx besides him and gluth has been soloed in naxxramas 25, gluth is not possible cause he hits too hard (you have no heals, only blood shield vs him) and he has too much hp\zombies heal him too much, but patchwerk would be possible if it were possible to outheal his hits in dps gear.
If no answer, next week I'll try to see if his hits are healable in 25, even in tank gear, although I would never beat the enrage.

I haven't tried personally, but avanger1304 soloed him on 25 and made a video; the solo is only for night elf blood dks (shadowmeld ftw) with at least 405-406 tank gear, cause at 25, like a warrior said before on this thread, he hits MUCH harder, 2 hateful strikes of 20-25k + his melee hits, which also are a bit harder than 10.
He has 13 millions, therefore it's only possible for dks with top DS heroic gear to do the dps to kill him before the enrage (6 mins), but it would be absolutely impossible to survive his melee damage in full dps gear, and in tank gear you wouldn't have enough dps to beat the enrage, so you must be a night elf and as soon as he enrages use shadowmeld, so he runs to his starting position, loses the enrage, and you re-aggro him.
Should save cds for the last 5%, which at 25 is like 650k, so not so fast to bring down, as he will hit even harder, his typical frenzy before dying.

Managed to 3-man as a blood dk (401), fire mage and ele shaman.
Kept myself up the entire fight by chaining cds (I'm the Death Knight) whilst they nuked the living hell out of him.
Flask, food buff, use all you have! It'll be needed if you want to attempt this bastard on 25-man mode.

Soloed on 25man on my blood DK at 85, post 5.0 (but before MoP consumable / enchants). food, fort scroll, no flask, real kings (from a dual-boxed pally that didn't touch the boss. And yes I got kings, not mastery + drums, so I didn't have more stats than I could get solo with a flask.)
The numbers were: about 42k DPS, 15k self-HPS (as measured by skada, so that includes blood shields)
I was also able to solo patch10 easily on my guardian druid. The new very-high-armor druid with sustainable self-healing works very well.
5.0 mechanics are interesting at lvl85, as str is now better than parry, and dodge is quite bad massive DR on dodge). Also, DKs now get some survival benefit from hit and exp, with the stacking buff to the heal on the next death strike, proc chance from melee attacks.
Had to use mostly tank gear, just a couple pieces with lots of hit/exp were worth it. Nearly full 410. For trinkets, I used 416 eye of unmaking, and 410 bone link (cleave proc scales with vengeance attack power). The STR on these gives maybe 4% parry over stam trinkets. I used fallen crusader on my weapon.
It took several attempts, some of the earlier ones in more DPS gear. I think I partly got lucky on the kill, and never had a drought of runic power along with a string of un-avoided hateful strikes and melees. Esp. not in the frenzy in the last 5% of his health. I think I was able to save a couple defensive CDs for that, but not much.
I used Conversion and Lichborne (heals for a TON with high end gear with vengeance, relative to a level 85 health pool. 1 death coil was more than half my HP, so I'd wait until I was low, coil once at the start of lichborne, and once at the end to finish the 10sec window at full health). I also used blood tap, so I could save up charges for more death strikes in the gaps between defensive CDs.
I also did a little bit of kiting as one other poster described, since he runs slow. You can occasionally get out of melee range for a second and not get meleed for a second or two. Maybe with a sprint you could cheese it even more; I don't think I was specced for Death's Advance at the time. Also I wanted to see if I could just beat the survival / dps check without cheesing it, so I don't think I kited for more than 5 secs out of the nearly 6 min fight.
I used Army of the Dead and my ghoul on the pull, probably they ate some hateful strikes for me.

Easy as a protection warrior at level 90, have the Second Wind talent, have your dps gear on, use Shield Barrier when you have enough rage, and use defensive cds at the end. Gives an awesome looking shield to transmog.

Relatively easy to solo as a mage at level 90 for those seeking the pet off Gluth, though it will require decent gear. My ilevel was 496 doing this.
The goal is mostly to blow the boss up before you run out of cooldowns. Thus fire is the better spec to play.
Open with all cooldowns / potion and time warp, combustion mirror images etc. Note that if you are in melee range of the boss, you
will
still be targeted by Hateful Strike even with mirror images up. I actually intentionally did this so the images wouldn't die as fast, as Ice Barrier just absorbed all of the damage and the boss's incoming damage was thus split between images and myself, giving me almost the full 30 seconds out of them. The boss was around a million health as images ended and died relatively quickly, but I was at about 30% health at the end.
Cold snap is an okay talent for this fight if you are lesser geared. Ice barrier is a must. Glyphed (evocation) rune of power is also pretty good.
The boss can be kited a bit as his movement speed is relatively slow, so you can blink / worgen sprint away to recover or get more damage in towards the end.

Patchwerk isn't too bad for a 90 ret paladin. I kept up Seal of Insight and used Word of Glory when I had to. I specc'd into Exectution Sentence, so I kept that up on me whenever it was up. He hits hard and fast, but all the healing the spec is able to do made the fight much easier than most classes will have with it. Took about 2 minutes to kill him with a 482 item level.

Soloed him as a rogue without any issues. Had to change a few glyphs (Evasion and Cloak of Shadows) and I used Expose Armor for the first time since I made the char probably, but it went down smooth and I had about 65% HP left. Preparation and Combat Readiness helps a LOT.

Easily soloable as an enhance shaman. Spec for Conductivity and glyph for Healing Storm, use Healing Stream Totem and Stone Bulwark totem, Maelstrom heal yourself if you get nervous. Was never in risk of dying.
Side note, even though you can bybass a lot of the trash on the way there, it will all pull when you aggro Patchwerk. Make sure to clear the area beforehand.

. As soon as you do, start backpeddling to avoid agroing (if you do, just sprint to a clear spot and hit Vanish). Once you're up there in Grob's area, its an easy walk out the balcony, across the pipe, and into Gluth's room thru the larger pipe.

Found an easy way to do this as a non-uber geared mage.
Was getting splattered with my normal pop all cooldowns and pray he dies before they're up strategy. So i tried something else: having the frost pent tank and it worked great.
Any standard frost spec should do. Have you pet tank and chain cast frostbolt on it. Once he got to around 75%, i started tossing in Frost Bombs, Frozen Orbs, and FoF and BF procs but was careful not to draw aggro off the pet. Took a minute or so to get him to 75%, but after that it went quick and the pet never got below 50%.
This was on the 10 Man version.

Soloed by 481 Ele shaman in 10-man. Stone Bulwark Totem, Fire Ele, pop Hero and Ascendance and go to town. I used a potion, Gift of the Naaru, and most of the last million health the only damage I did was with Lava Surge while spam healing myself.
Still no pet from Gluth though :(

I just soloed 25 man as a 90 Prot Pally (490 ilvl). I went at it at first as ret and got destroyed, no way to keep up with his insane damage. The next pull I decided to see if I could even have a shot at it as prot. The damage was manageable, but I was 18% short of the hard enrage. My third pull I flasked, got a str food buff, and potted in addition to putting on ~5 pieces of my best ret gear. I beat the enrage this time by around 30 seconds, so it is possible!
If any non tank specs are able to do this, I'd be very very impressed.
Some more details about what I learned over my 3 pulls specific to prot paladins:
1. Refresh weakened blows debuff with hammer every 30 seconds, this was the difference between having to wog myself and keeping myself up mostly with seal of light.
2. Pop wings/holy avenger at the start and again in the middle (the second round, I WoGed myself up from about 20% health to full).
3. Shield of the Righteous as much as possible, try not to use WoG unless you get in real trouble.
4. Execution Sentence on CD.
5. Keep Holy Shield up at all times.
I shield walled at 5% frenzy, never used ardent defender, bubble, or lay on hands, so there is definitely margin for error.

As a holy paladin i usually switch to ret when farming old content, but this expansion i haven't been able to get gear for ret since it's my offspec so i lacked dps to kill this thing before it killed me.
After a couples of tries dying as ret, i switched back to holy and it was really easy, extremely. It took me close to 3 minutes but i was never in danger of dying and didn't do anything special.

Just soloed him as a rogue (10 man mode) and almost killed him in 25 man (currently i have 468 ilvl). However i found a very interesting mechanic to help me in such situation. I got a

trinket and it helped me a lot! When you are at low hp just vanish and your companion will take over aggro , he will live long enough for you to get full hp and even some cooldowns back. Just remember that his off hand strike seems to ignore evasion ;/
Anyway he will be soloable in the near future for all rogues, once you get better gear (i could probably reforge for expertise)
So remember - if you are rogue this trinket will help you tremendously. Just a shame you need to wait 1 hour :(

For 10-man Patchwerk on a level 90 destruction warlock:
Use the voidlord, and make sure that you are NOT using the "Health Funnel" glyph (which instantly restores 15% of your demon's healthy, but has a 10 second cooldown). Again, do NOT use the Health Funnel glyph - the 10 second cooldown is too long for this fight.
Stand well away to make sure you're not getting any aggro or damage, use Curse of the Elements to increase the (very little) damage you're going to do, send in your demon and basically alternate between Immolate, Rain of Fire, and Health Funnel. When Immolate or Rain of Fire drops off, re-cast, but otherwise keep casting Health Funnel. It will drain your HP, but you should be able to gain enough Burning Embers from Rain of Fire to alternate between Ember Tap, Drain Life (if your Voidlord is close to max health), and Healthstones. Always keep one Burning Ember up in case Patchwerk somehow kills your demon - he shouldn't, but you want the instant summon from Flames of Xoroth.
Patchwerk can put out a lot of damage to your demon, so keep using Health Funnel during the whole fight. The only time you shouldn't be casting Health Funnel is to re-apply Immolate or Rain of Fire, or to use Ember Tap or a Healthstone.
It will be an easy fight with this technique.

As a mage 10 man is doable even with bad gear. Just spec frost and let your elemental aggro. Once he is attacking your pet you can't pull aggro on Patchwerk as long as you stay out of melee range ( he only attacks people in melee range) So feel free to nuke as hard as you can. Keep an eye out for your pet's healthbar and cast some frostbolts when getting low. (This will happen rather quick and a lot ). If you do more then 13k DPS you are fine

Just solod as a 90 warrior on 10m. First go I went in my dps gear (ilvl 477) and had him to about 10% and died. I probably could have gotten him if I'd tried a bit harder and used my cooldowns more effectively.
Instead, I switched to my prot gear (ilvl 456) and spec with Second Wind. No way he could ever burn through that so killed him no issues.

Just soloed as a level 90 Fury Warrior in Dreadfull/Malevolent gear with 2 heroic swords.
HP went down relatively quick but there's no way on Earth he will get through second wind when he isn't enraged. Just kill him before the enrage ;)

Since it hasn't been updated for hunters since 4.0-
I just beat 10m version with a 458 BM hunter after about 4 mins. Talented with Spirit beast for the 2% regen, which really helps, as well as glyphed with Animal bond for extra healing boost. I had a spirit beast (who can heal 60k hp every 30 secs as its racial) in Tenacity spec and with growl enabled. The trick to this fight is that the debuff he applies to you only when you're in melee range, so have your pet tank him while you fire away from a comfortable distance. At level 90, your mend pet ability can heal your pet faster than he can kill it, so make sure to keep it going and constantly renew it. Otherwise stay away from abilities that'll cause the boss to aggro onto you, like Bestial Wrath, and he should go down pretty easy. Good luck!

Soloed him on my Monk today after a few sloppy attempts (Windwalker, ilvl 484). Trying a straight-up out-DPS race didn't work -- he still hits like a fleet of trucks -- so I focused less on DPS and more on healing abilities.
Ultimately, I ended up using Fists of Fury for the bulk of my DPS, spending most of my energy spamming health globes and using Expel Harm every 15s to keep up with Patchwerk's damage. I also kept a health pot in reserve for his final 5%, where he really goes nuts (I actually died with him at 1% on my first attempt)
I think it took about 2 minutes total, longer than soloing most Naxx 10m bosses, but still plenty fast enough for the achieve. I think this is one of the more interesting soloing challenges at 90 (depending on your class), so GL to anyone tackling it. :)

easy to do 25 as a hunter. go in with 2 turtles, keep mend pet up 100% of the time, let your turtle run through shell shield, then use last stand when it gets below 30%, shell shield should be up again shortly after last stand. when your second shell shield ends, dismiss and call your second turtle who's cds will all be up. depending on your gear level, you may not need your second turtle.

Only really need Glyph of Mending, and a pet with Shell Shield.
-Keep Mend Pet up at all times. DPS as much as you can. Your pet will die eventually. Cast Deterrence and Revive Pet, Feign Death to let pet get aggro. Misdirect pet and continue DPS/Mend Pet. Somewhere in there before your pet dies again, use Readiness (so Deterrence is back up). When your pet dies a second time, Deterrence and Revive. Patchwerk should go down before your pet dies again.

Done as a frost mage, ilevel 459, but was not nearly as easy as the comments said. On my first attempt, I just unloaded DPS on Patchwerk, but the water elemental dropped like a fly and Patchwerk was on me. Needless to say it was a short fight.
The second attempt was a bit different, after waiting until my elemental got him down to 95% solo to maintain aggro, I unloaded DPS using Mirror Image, Heroism, and Icy Veins. I kept myself on my water elemental, constantly throwing frostbolts to it to prevent its death. When it would reach 100%, I would swap to Patchwerk and quickly throw a frostbolt and whatever Ice Lance/Frostfire Bolts I had acquired. This went on for about six minutes, as I threw my last procced Ice Lance the enrage timer for 10 man kicked in and one-shot my elemental. This was the only way I've been able to solo him.
TL;DR - Don't go all out DPS, focus on healing your elemental.

Another hunter strategy here using a spirit beast (25 man Patchwerk). I didn't feel like going out and taming some turtles so I used my trusty old bear spirit beast with an obvious tenacity spec. My ilvl was 498 (raid finder weapon/gear).
Keep Mend Pet up constantly and Spirit Mend your pet on cooldown (use a /cast Spirit Mend macro so you can just keep dpsing without having to change targets).
The important thing to do is to Cower on cooldown reducing damage on your pet by 40% for 6 seconds. It's nice to have Cower available for when Patchwerk goes beserk, although I'm sure you can burn down the last couple percentages if your pet goes down (just make sure you're at max range to give yourself enough time). I feel as if Cower is a lost ability that not many people remember (or remember it as a threat reducer).
Use Last Stand when you feel your pet needs the bonus (or save it for the berserk part if you find no other useful moment).
Your pet will be (near) 100% during Cowers and stacked Mend Pet/Spirit Mends. Didn't need to resurrect my pet at any time (if you do, adopt the Deterrence, Revive Pet and Feign Death strat mentioned below).

Managed to solo this guy on 10 man as an ilvl 500 Assassination Rogue. Even at level 90 he still hits like a truck.
Poisons used: Deadly and Leeching.
The way I did it was a fairly typical tank and spank dps race type fight. Popped all my cooldowns at the beginning of the fight (Vendetta, Shadow Blades, Lifeblood (herbalism) and Evasion), and used recuperate after the first 5 CPs.
After that, it was a standard DPS rotation with the exception of (glyphed) Shiving on cooldown to slow the loss of health. At around 60% my phone rang (brilliantly timed mate) and I had to step away for a few seconds, after which I popped Preparation and Evasion again.
Finished the fight on around 30% hp. So he's tough, but I think Brutallus is tougher (I attempted him a while before, and although I had worse gear, I only had to fight until about 90% to see that it was going nowhere).

Just solo'd on 10m as a BM Hunter (iLvl 502)
I forgot that when you aggro Patchwerk if it hasn't been killed -
everything in the room
(except the oozes that run across the floor)
will aggro onto you.
Had him at about half-health by the time the mobs reached me (was fighting him at the end of his pathway). I popped everything I had to burst him down (I was using a Basilisk pet). I had the Glyph of Mending to help him stay alive, was able to down Patchwerk while getting mauled by the mobs - however I did FD after I got to about 40% health, and let the mobs aggro onto my pet.

Solo'd on 25 man with 531 Guardian Druid. He barely even tickled me, only dropped below 95% hp once and that was because I alt-tabbed to change the song I was listening to, and thus didn't have Savage Defense up for a few seconds.
I found this amusing because in the previous patch he slaughtered me on 10 man, absolutely no contest.

Time Taken: 3 minutes 10 seconds.

Soloed as a 551 arms warrior on 25 man and realized afterward i forgot to unequip my kirin tor ring -_-
Gonna want glyph of mortal strike and die by the sword and a third of your choosing and talent second wind and bloodbath. pull with shattering throw and pop your big all in one cd macro and use slam instead of overpower for the duration of recklessness but still cast MS on cd so you build up 5 stacks of overpower. once your cd's are blown fight like normal until he gets you under about 150k health and hit die by the sword and just MS and OP spam until you run out of OP stacks, this should extend DbtS duration quite a bit essentially making you invincible for a good long while.
once DbtS wears off you should have him near 20% health, at this point use defensive stance and hit remaining defensive cd's as needed until he's down. you should be kept alive by second wind at this point barring really bad parry rng.
The fact that we can use shield wall with a 2 hander now essentially makes this possible but it will still be close.

Solo'd rather easily with my level 88 protection paladin. Seal of Insight + Eternal Flame and my health never dipped below 95%.
As others have said, be sure to clear the trash or else you'll get some unwanted company mid-fight. Even so, Holy Wrath + Hammer of the Righteous + Consecration will take care of any adds that may come along.

Soloed as 90 Demonology Warlock.
Moderately difficult, and my solo attempt happened after wiping 6 or 7 times previously.
What's needed is a disciplined, sort of monotonous approach where you just rely on a few things, and wait him out.
Wait him out until his enrage timer pops!
Of course, as Demonology, you'll start by siccing your minion on him.
Stick with the siccing for a long, long, time. Somehow Patchy drains a lot of your health too, so plan on use of Healthstone & Potions at appropriate interval.
That's the first layer of bringing him down. Of course, you also want to make sure that Corruption & Curse of Enfeeblement stay up all the time. Additionally, consider once in a while turning into the devil and applying the Doom curse.
Patchwerk drains both you and your minion quickly, so scan all your vitals to see if it's more appropriate to use Potion OR Healthsone OR Health Funnel. Top hint, most of the time you'll use Health Funnel. But,
watch out on your own health
. You don't want this to disappear while you are paying attention to your minion.
Heavily consider waiting on use of 2nd pet (Grimoire of Service), and 3rd pet (Summon Doomguard) until Patchy considers himself "enraged". Enraged happens at about 6 minutes into fight, as evidenced by all the posts here. During "Enraged", Patchy will drain health of your minion especially rapidly, so be sure to have Health Funnel on once you notice the drain.
Apparently, at "Enraged", Patchy takes a bit more damage, so he will then go down quite quickly. Your extra pets/minions should help.
Sorry, it sounds like more than it actually was. But this is how I soloed Patchwerk at Naxx.

Trying to solo in heroic as a 557 elem shammy (mostly resto gear), going toe to toe & blowing all CD's I get him to 3 million from 13 million Another try 4 million. I don't see much point kiting, as he's faster than I am.
Bit of a half-assed try. Got to go back with potions & health stones.

Drop Fire elem & flame strike.
Lust, stormlash totem followed by a macro'd Ascendence, Shamanistic rage & Spirit walkers grace, then spam Lava blast, with the following healing rotation :
Healing stream
Healing tide
Healing stream
Call of the elements & healing stream again.
After ascendance expires :
Elemental blast & Lava Blast, followed by 6 Healing surge - rinse & repeat.
Except you will be OOM in nothing flat.

Somewhat tricky but possible to solo for a ilvl 530 Shadow priest on 25man, if you prepare accordingly:
-Glyph of Inner Fire for more armor
-Glyph of Renew for more hps
-Glyph of Prayer of Mending for more hps
As well as Angel Feathers, Divine Star and Divine Insight talents.
To survive tanking Patch to the face you will need to keep everything healing related on cooldown, shield, divine star, PoM and Renew always ticking. You will want to adjust your normal dps rotation to maximize Devouring Plague uptime and recasting it at 1 Orb as soon as it runs out, the 1% health/s is very helpful. Divine Insight helps you generate more shadow orbs to increase plague uptime. Don't even bother trying to Flash heal spam, you'll just oom yourself in no time.
Sooner or later you'll be low on health, that's when you'll want to Angel feather kite Patch all the way to the other end of the long hall he walks in, all the while keeping renew and ideally plague heals ticking on you. Rinse repeat until he gets to enrage, just pop Vampiric Embrace and finish him off.

Soloed this boss on 25 man mode on my lvl 90 ilvl 535 ret pally, I had a bit trouble, but it was easy overall.
I was healing myself with word of glory every time and dpsing him as much as I could, popped my defensive cds (bubble) when he went on 70% hp or so, then I popped my offensive cds, got him on 5-15% hp, survived as long as I could, used lay on hands at the end, he was pretty much dead by then, but he did a lot damage when he frenzied, I popped my wings again and killed him with full burst (didn't heal anymore 'cause he was dealing more damage than I was healing).

Solloed on 25 man with a ilvl 510 lock. Tricky for sure. Do your usual rotation and make sure to keep healing up on your void. About halfway through the fight, your void will die. Quick rez him with Flames of Xoroth, soul shatter and he should grab aggro back. I did not produce alot of aggro using CB/Dark soul instablitity until the void died. Then go nuts, and keep him healed, bring out your terror guard.

Soloed as a ilvl 510 ret paladin
Did my normal rotation except where I would normally do Templar's Verdict I hit Word of Glory instead.Used seal of insight and kept sacred shield up as much as I could. Managed to kill him just as he hit frenzy.

If you aren't careful, this guy can seriously own you even if you are a fully SoO raid geared lvl 90...at least on 25 man. I was doing a battle pet run, my 550GS 'lock had brezzed through all the other wings of Nax without even breaking a sweat or dropping below 1/2 health. I ran in attacked Patchwerk and got my a** handed to me in less than 30 seconds, (part of the reason is that I was so overconfident in my abilities, I didn't even bother to have my Voidlord summoned....)
So this is what I did on my second run, I maxed out my burning embers on the trash, I sent Voidlord to attack Patchwerk, with me standing just in spell range, gave VL a couple seconds to get aggro, then popped all my cooldowns and trinkets, and let the Chaos Bolts fly. This alone was enough to bring Patch down to around 50% health, but by the last CB he had aggroed me and was almost on top, I hit him with a couple Conflagrate/Incinerates until he started cutting into my health, then I popped my Soul Shatter giving aggro back to VL and moved back to just in spell range. By this time he was down around 40% health, so for the rest of the battle I just played aggro tag with VL, when VL would start getting low on health, I would pull aggro and use burning embers to heal. Only took a couple minutes with this method.
(As a side note, the reason I had underestimated him the first time is that I had killed him easily before with my Brewmaster Monk, the Brewmaster's lvl 90 damage mitigation/avoidance makes the fight a trivial tank and spank. I'm guessing all tank classes would have similar results. So if you are running a class that has a tank spec, you might try switching to it before this battle.)

Not sure if it's been said , but I figured I would give my two cents on this guy.
He hits like a truck
As has been said by a few people in afew different places. Patch hits harddddd
Then again, I'm a lvl 9o warlock with a ilvl of 492.
Anyway!
Sent my voidlord in to be a tank, y'know, keep my ass safe.
On average. The damage done was
15.5k
. The lowest that I saw was
13k
.. May have been lower, but I just didn't see it.
Anywho, he's easy, really. Hits hard though. Like... holy #$%^. DX
