As of patch 10.1.7 this is back in the game and dropping off Rattlecage Skeletons like before.

Spent about 15 minutes running around killing every mob before it dropped, so yes it is tedious. Killed all the ghouls too as they seem to share a respawn with skeletons so helps refresh skeleton mobs. Many wolves and bats got killed in the process lol

Dropped at around 1 hour farming together with 3 / 4 Deadman Weapons ! Dont forget to kill the Ghouls aswell because they share spawn with the skeletons.

Not sure if this is just RNG, but didn't get it after an hour on another character, then created an undead to do the intro questlines and had two drop in less than ten minutes.

Got this after ~100+ kills, however worth noting that this was obtained on a max level character (level 80 at the time of writing this). Doesn't appear to be limited by level!

🧠 Side note:
I’m currently doing a personal challenge called “Level Locked”, where I lock my XP every 1–2 levels and treat each level as its own endgame — collecting everything and fully completing zones, quests, professions, and rare drops like this one before progressing.
It makes even low-level content feel meaningful again and turns farming items like this into a real progression goal, after having collected everything of that level I must fully clear the corresponding dungeon and treat it as solo endgame like we used to look at Molten Core, when done I can progress to the next level and continue my journey in the same way.

🎥 I’d also love to follow a YouTuber or streamer doing this kind of challenge, as I don’t have enough time to record it myself — if anyone is doing something similar, let me know! I can send you the lvl locking layout.
