Found it on a body at 34.81 in ghostlands

The corpse is named Blood Knight Dawnstar. You speak to it as if you were speaking to a normal NPC.

34.81 right next to the chained cauldron

You get this from Blood Knight Dawnstar, a dead blood elf in Darkholme at 34, 81.

Pick this insignia up at 34.9, 81.9 by searching the corpse of Blood Knight Dawnstar, in Deatholme in Ghostlands. Follow the Dead Scar to Deatholme and go directly south of the entrance to the base of a chain cauldron's stairs.

It appears that you can no longer get this item. Which is a shame, because it'd be great for RP if someone wanted a Blood Knight character. I know I do : /
