I have this and upgraded to ilvl 411. It hits for minimum 179k but sometimes as much as double that. I don't know why or if you have to stand very close to target but it is a pretty nice trinket.

Here is how you farm this trinket. It is actually best to do this in a group because the trinket drops only from rares. To spawn these rares you need to collect wards and after 5 of a certain ward you go to the altar and use them. This will summon a rare and you can kill it for a chance to get it. So having a group means more people will be getting wards and there will be a constant flow of rare spawns. Also make sure to do your weekly chest since it's right there and I believe you need to get 3 fire keys to open. Don't remember what those keys are but it gives 402 loot that can be upgraded to 430.

You have to be very close to use this, I tested on a target dummy and the range was about the 1 1/2 of my shoulderwidth.

I've max levelled this to 411 8/8. Hits for 188,323 fire damage. On my rogue I can apply Cold Blood with a 100% crit chance, taking damage from this trinket to 376,646 every 90 seconds.

Can be extremely powerful single-target, especially when it crits. 480 base damage is 366,149, and my crit did over 770,000.
