To get most value out of it, you place the target reticle manually on players, e.g. on the melee camp.

As alternative, if you dont like to manage another key/binding, you can use a macro to combine it with any other spell. The trinket area will be placed centered at your own position. Ideally, you use a spell with 2 minutes cooldown.

Holy priest here. I got this in my vault this week and I'm liking it quite a bit. We don't have much in the way of damage mitigation, and this is really nice to throw down to reduce heal pressure.
