From what I've seen using it in Plaguefall, the shoulders make you immune to 真菌猛攻者's 真菌风暴.

I had the buff for like 2 weeks, i was legit worried my character was bugged, turns out it was this piece of gear, lol

Something that no one mentioned yet. If you have these equipped, anytime you zone into the world, or land from a flight point. Your character audibly farts. I'm not sure if other characters can hear it, but you will hear it if you have sound enabled.

Edit 2022-04-05: I think they nerfed the sound effect with this item, because I haven't heard it in a while.

FYI, if you convert these shoulders to Tier in the Creation Catalyst Console, they lose the 恶臭 Equip effect. I don't expect anything to come of it, but I reported it as a bug just in case this isn't intended.
