Next quest: Maintaining Focus.
It's either bugged or it changed when it came to live. I didn't drop the Void Focus recipe, instead I dropped 6 of these, which is the cap and when you use it u get a Void Focus which has an use effect: Unlocks a Cursed Coffer that I have no idea what it is. And the first Void Focus I got it disappeared and I had to farm again.
Ok, This is related in some way to the possibility of crafting 470 gear.
This is the full explanation
First, go to any of the Assault zones and kill mobs, from any random you will end looting Recipe: Void Focus
Continue killing mobs in assaults to loot 10x Void Focus Splinter
, droprate is not a cake btw.
While farming mobs, you're also looking for your Profession patterns. If you've collect 10x
and haven't gotten your patterns,
keep farming even more N'Zoth mobs!
Remember to take use of all your available time inside
to kill mobs as in those scenarios the drop rate is suposed to be good.
Second, you will start a serie of quest.
After crafting Void Focus
you will start the quest
Something in Your Mind, just hand in and NormalLevelGear will
be unlocked for craft to you.
If you wonder how to "craft" your void focus, you will find it as a new craftable item when you open your profession tab. It will be under the
category of "Focus"
After you craft your Void Focus and complete the quest, it becomes an item similar to as anvils are for blacksmithers. That item will be in the
Chamber of the Heart behind Wrathion. You need to be by the Void Focus to craft your various profession items.
Next two quest are to farm
You will start with
to loot x20 that will unlock HeroicLevelGear to craft and later you will continue with
to loot x200
that will unlock MythicLevelGear to craft.
As the tooltip says Shred of Insanity can be looted from various activities ingame: from bosses in Ny'alotha and World Bosses, obtained by
winning Battlegrounds and Arenas, or from Honorable Kills.
World boss and Normal raid boss will grant one
per weekly kill
Heroic raid boss is 10
per weekly kill and Mythic raid boss 20
Arenas and Batlegrounds has a % to come
as reward after a win.
World PvP - Low chance from a Honorable Kill.

Farming these inside lesser visions seems to be the way to go, I farmed out in the world for 30 minutes and only had 2 to show for it, went and did my lesser vision daily and got the other 8 within a minute or two.
If you've already done your lesser vision for the day without getting the 10 splinters, you're not screwed: although the drop rate for these are much higher within the lesser vision, farming regular mobs out in the world in the assault will still have this drop.
Void focus splinters seem to drop a lot from lesser vision mobs alongside the crafting recipes. I got like 5 before leaving my lesser vision, so I had to farm the rest on the current big assault (Uldum this week). The splinters and the recipes to drop a lot from mobs on top of the bridge at 53 44 (near the raid entrance) from Acolytes of N'zoth and faceless mobs.
Has anyone been able to craft two Void Focuses? It looks like the first one is consumed when you complete the quest it starts (take it to Wrathion), leaving the player unable to craft the 440 gear that requires a Void Focus. I tried farming more splinters after losing my Void Focus but after about 20 Faceless and K'thir mobs it didn't drop and I gave up.
8.3 update (today, week 2) the void relic splinters drop from mobs in Uldum! Well technically they drop off the area where the corruption has moved to! Aka, the raid entrance, and the weekly assault. In Uldum this week and the mobs dropping are. (also in the daily vision: Lesser visions of N'zoth.)
Void relic splinters: K'thir Doomslayer, Acolyte of N'zoth, Black Emperor Beheader. 50,40. Mir'at. Then along the Vir'naal Dam, 54 40ish. This is where I found all of mine, in those 2 spots. I'm sure there are others. The Faceless elites on the Dam probably drops them, but i never got one from them, so I can't confirm that.
A decent place to farm in Uldum is at 62, 72 where there is a mini-event with K'thir and cultists. Farmed them all pretty quickly, the event respawns in about a minute after it's completed. Just be sure to put out the candles after you kill the mobs. Also drops relic fragments at a good rate.
If you didn't quite get enough
Void Focus Splinters
from your lesser vision in Uldum (best place to farm them), the exact same mobs (all that are listed in it's tool tip) are in the same place outside of the vision around
Faceless, K'thir, Aqir
can all be found in that area and farmed quickly for the rest.

How to unlock the Void Focus
1. You have to loot a Recipe: Void Focus from N'Zoth assaults or Lesser Visions. It's a random drop from Faceless, K'thir, Aqir, and Cultists mobs that worship N'Zoth.
2. Use the recipe, then farm the same mobs for 10 Void Focus Splinter. (These same mobs drop the rank 1 for the new raid recipes, so you should keep farming them until you get the recipes.)
3. Open your profession window and craft the Void Focus. This will start the Something in Your Mind quest.
4. Go to the Chamber of Heart and turn in the quest at Wrathion which unlocks the Void Focus. Now you can craft ilvl 440 gear.
You will automatically get the first upgrade quest once you finish the first Void Focus quest.
The first upgrade is called Empowered Void Focus, it will let you craft ilvl455 gear. You just simply have to complete the quest Maintaining Focus by collecting 20 Shred of Insanity
The second upgrade is called Dreadful Void Focus, it will let you craft ilvl470 gear. You have to complete the Dream Catcher quest to get this upgrade. You will need to collect 200 Shred of Insanity to complete the quest.

Don't want to steal any comments, therefore i'm reposting one of the comments i hardly found among the multitude here, so for those trying to FARM these Void Focus Splinters AGAIN, after turning in the quest, you don't need to!

P.S. Just wanted to highlight this specific info as i was farming them pointless after i already got the cauldron and didn't understand why they don't drop anymore.
For anyone who has completed the quest for the first 10 & can't seem to find anymore to craft, you have already created the void focus.
Just behind Wrathion in the Chamber of the Heart - you'll find the cauldron which is called
and you stand in front of that to craft your items.
I was confused about this for many weeks. I can confirm that you have to learn two crafting professions to max skill to decently increase your drop rate.
I'm just going to say it since nobody else seems to have said it:
Poor mechanics, poor drop rate, not much was explained about this item to begin with (in-game). And after finally getting enough to complete the quest, it feels like I just wasted all that time farming.
Well, time to level up professions and hope for a better drop rate and try again I guess....
Haven't seen this yet so... er, blizz, why no path for this for inscription? Every other crafting prof but not that. I'm confused... scribes can craft many things where this would make sense.
Want to mention that Aqir Impalers have an incredibly high drop chance for this item. Farmed 10 of them in less than 15 minutes!
In order to get these to drop, you first have to have gotten a drop,
from invasion mobs. I must have gotten it, but I didn't notice, and probably realized it was a recipe and clicked it without actually looking at it, at some point when my bags were full of trash. I didn't even know what the void focus splinter was - it has the same icon as the Void Relics that drop, that you use 6 of to make a key to a bigger chest.
once you have 10 of the splinters, and have forgotten that this was a recipe (or not noticed), you need to go into your crafting interface and *craft* a Void Focus. Only THEN will you get the quest to give back to Wrathion. I didn't see the Void Focus behind him till I took the follow-on quest,
if you're Horde) to make an Empowered Void Focus. Which needs mats from the Ny'alotha raid, world bosses (I assume BFA ones) or PVP. That's the step I'm on, so I don't know how that will change what you can make at the Void Focus. I only have 1 item I can make there. (JC=Ring)

Little Aqir swarms are the best way to farm these imo.
gave me the most of these in Vale of Eternal Blossoms. I bounced between two large groups at 64, 44 and 64, 48 and flew around killing other things, but the Zealots gave me pretty much all of what I needed in about an hour if not less.
I was able to quickly farm my splinters and patterns at the Tu Shen Burial Ground in the Vale. I avoided the faceless and just killed all the humanoids I could find. This is the same area where you'll find
. Hit all the ramps leading up into the burial grounds and you'll never need to wait for respawns. The humanoids are easy to gather in groups and AOE down.
