I have run Molten Core enough to get 2 characters to exalted with Hydraxian Waterlords and am yet to have these drop even once.

This is part of pve Tier 1 PVE raid set 塞纳里奥 .All part of the set can by looted in 熔火之心 lvl 60 raid , which is located beyond 黑石深渊 in the deepest part of 黑石山.

These have finally dropped for me and gave me the appearance achiev.
Has taken many, many, many runs.

THis item ONLY drops from TRASH. And is BOE so you need to equip to get achievement.

If this is the only item you are chasing then best way is to just kill the trash and leave bosses alive. Then join any group and then leave it. You will get teleported out (but not out from the mountain) after a minute or so. Reset the instance and go again. Repeat etc.

Just take care not to kill a boss with AOE!

I run Molten Core cca 50 times (and being exalted etc) and not having it (and belt neither).
But for this, you can just run it and not ouch bosses - and then again and again and again to get this.
