Ghoul Control Issues - Unholy

So... I'm leveling a DK again, currently lvl 70, Unholy spec, and I'm finally not terribad. I'm consistently doing first and second in dps in dungeons, and almost always topping recount in damage done. (Without heirloom gear on this server. *cry*)

I'm loving the Unholy spec and enjoy having the permanent ghoul. (Makes me feel less lonely when I'm soloing.) However, I have a problem with the ghoul running off and pulling groups that the tank hasn't gotten to yet.

In instances, the ghoul is ALWAYS on passive, with leap and gnaw turned off, claw left on. I also have the /leap /petpassive macro handy for when he tries to run off. When I use it, he returns to me, but then will run off again, causing facepalm/tank aggro.

I DO use a pet attack macro: /cast plague strike /petattack... Is that my problem? Am I unintentionally letting him off the leash? For some reason, I thought that only let him attack the mob I'm currently meleeing. Or, am I accidently spamming my pet attack macro after I recall him? Or something else?

Am I simply not having enough situational awareness?

With my Ghoul I have him on passive and then have his attack on binded as Alt-1 , so when ever I click a target I do the bind and then start my rotation, then on the next target the same again. After a short while it becomes part of your rotation and you do not realise you are doing it.

Apparently, in an effort to not be fail, I gimped myself.

Ghoul on defensive, no attack macro = zero unintentional pulls in three goes through Utgarde.
