This achievement is most likely the follow-up we all were expecting to 这下肯定再也找不着了. I haven't done the fight yet but seeing as we will be fighting a "Fleshy Monstrosity" and are going to be returning it's meat to "The Emptiness", I think it's pretty safe to say this achievement is somehow connected to the Sarkareth fight in Aberrus :)

To complete this achievement before the final boss near the teleporting orb is a small puddle that grants a debuff, you must get 10 stacks of the debuff to get a lump of flesh, which gives you an extra action button to throw it and lose some hp. Once you kill the first 3 minibosses Alleria will toss you up in the air and pull you to the center. Walk towards the center and toss the lump of flesh in. Once everyone throws the lump of flesh in a fleshy monstrosity will spawn. (Focus this mob as he has a convert mechanic that makes you dmg those around you.

Right after you defeat the Lightbinded Vanguard, there’s a blue/black blob on the ground, all players need to stand in it gaining stacks of 缠结虚空. Once you get 10 stacks, you’ll get a debuff – 血肉肿块. You’ll also get an action button to use later on during the Crown of Cosmos boss fight. The debuff lasts for 10 minutes so, act fast.

During the intermission phase when players are being pulled to the middle, players need to get close to the middle, the action button will pop up, all players need to use it. After all players throw the flesh and the intermission phase ends, a 血肉畸合体 emerges from the black hole. Defeat the add and continue the fight. During the second intermission phase, go close to the middle again and all players need to use the action button. This will return the flesh into the void. The achievement is now met, and you can continue the fight and defeat the boss to get the achievement.

once you get the "lump of flesh" buff, you cannot use any immunities (i.e. cloak of shadows, divine shield) between when you pick it up and the intermission where you use your extra action button, as you will lose the debuff and therefore not have anything to throw into the black hole.

Can confirm that if a member of your group dies after the SECOND throw (aka on the last phase), the achievement won't be failed.

There are not many comments, so I will chime in. After you kill paladins, click to spawn the portal. Before taking the portal up, you will see a puddle on the ground. Everyone in the raid must stand in the puddle until you get to 10 stacks which gives you a buff. Now go up and fight Cosmos. At each intermission (when you are being pulled in and dodging lasers), everyone in the raid group needs to get close to the middle until the extra action button appears. Click it to throw the meat. In the first intermission this will spawn a mob you kill. After you kill it, everyone will have the buff again. In the second intermission, everyone repeat the throw to get rid of the buff. If you are tracking the achievement it will change from red to white when done successfully. No one can use an immunity. No one can die until the second throw is completed. Dying afterward is fine.
