Have players position on the 6 red circles (there are 5 in a half circle in the western part of the platform and 1 in the east part of the platform) in phase 4 and dispel the Manic Dread fear while they are on top of the circles so that they drop the Caustic Delirium eyes on top of the red circles, when all 6 red circles have the Caustic Delirium eyes on them a portal to the secret cow level will spawn in the middle of the room and inside you need to kill the Twinklehoof Bovine quickly before the portal closes. Save Time Warp / Bloodlust / Heroism for the cows.

To open a portal to the secret cow level you have to power up the 6 "runes" on the ground. To do this players must stand on top of them in Phase 4, get afflicted by 癫狂惊惧 and then get dispelled. The 腐蚀谵妄 on the ground will power up the runes as you can see below.

Once all 6 runes have been powered up a portal will open in the middle of the room. Press the extra action button and you will be teleported to the secret cow level! From there you have a limited time to defeat 10 烁蹄牛.

This achievement is trackable so when the criteria has been met you can kill the boss and get the achievement.

Fun fact! The groups going for World First 永恒团队的荣耀 have used the 乌达维斯的晶化之眼 and found six invisible npcs (turned into demons due to the toy's effect). This has helped a lot in discovering how the achievement works.

There are 10 cows to kill in a very short amount of time and they're pretty spread out so doing this achieve with just 10 people (as we tried) is very hard. Bring lots of friends.

We had a small raid group of 12 and were trying for this and it was proving difficult with the cows so spread out. We finally solved it by having everyone just hit a cow so that it moves towards the middle where we could then have an easier time nuking them down.

This achieve is not possible with any less than 7 players.
1 tank to keep agro on the boss
6 players to stand in the runes
Runes must be powered up at the same time, you can't power up 5 and then do the 6th separate (this might not be true, we had 4 players doing runes and couldnt activate the Secret Portal)

This achievement is easy doable with 3 people (can be done with 2 but needs more time).
We did it yesterday with 3-man (Resto Druid, Holy Priest And Retri Pala)
The big trick no one mentioned before is that you can use pvp trinket to remove the fear.
Our tactic was :
1) Pull boss and bring it below 50% (idealy 45-49%) so the boss will start cast Manic Dread.
2) On first fear (Manic Dread) Paladin used trinket, Resto druid trinket himself and dispelled the priest
3) On 2nd fear, paladin used bubble, holy priest used trinket and dispeled the resto druid.
4) Enter the portal and kill the cows
5) kill the boss and get credit for the achievement.

Ice block should work too as well, didn't have the time to check hunter's turtle.

The achievement should be doable with 2 immunity class (Mage/pala), or with 2 dispel class (shadow priest/ lock/ any healer). /you just need to wait 2mins for trinket to reset

Did this achievement (relatively) quickly with just 2 people- shaman and a rogue. Shaman used tremor totem just before each Manic Dread finished casting, which instantly cleared it while standing over the eyes. On the last cleanse, rogue rushed into the portal and used every movement cooldown and damage cooldown to kill all 10 cows before the timer ran out. It did take a few attempts to really perfect the solo-cow run, but it's doable! We finished the entire meta with just 2 people. Roughly 421ish ilvl (both) at time of completion.
