It's the exact same game as a Normal realm, except the playerbase typically consists of more people wanting to act out their characters roles; i.e. speaking in character, staging events for role play purposes.
Normal realm conversation
: "Hey Dave, 'sup?"
RP realm conversation
: "What ho! Do mine eyes deceive or doth the valiant Trevorlim of Stormwind stand before me! It has been too long friend, too long indeed. Please, seat yourself by the fire and revere us with a tale of thine recent adventures. Havest thou slain the Ogre you saught in that sorrowful swamp?"
(continues on like this with the inclusion of many, many custom emotes)

I'm not sure why people always seem to think that old English = RP. There are lots of random, silly accents (trolls, dwarves, etc.) but old English? What NPC talks like that?
Long story short - on RP servers, you pretend that you are your character. Play an orc warrior? Then you talk like an orc warrior, try to decide what he would do in any given situation, things like that. You're basically acting out a story for a character that you make up and interacting with other people's character's. Not a hard concept, and obviously not for everyone. ;)

I still dont understand how people cant figure out by themselves what is role play... it must be the same kind of person that believe MacGyver is a real person.
